10/31/94: Alleger, a welding supervisor, allegedly received a negative performance appraisal and was then removed from a supervisory position. Alleger believes that these actions were the result of working for a new supervisor who is not orientated towards a nuclear quality environment.

Alleger believes he was viewed unfavorably by the supervisor when he stopped welding work on an instrument tubing line (DEH system on turbine deck) because the procedure had to be amended before work could continue.

12-15-94 Telephonically interviewed Hallenbeck re: letter he sent in about Ernie Poarch and got additional names referencing the first interview. See Report of Interview. Hallenbeck said Poarch's Turkey Point supervisor was Joe Marcheise who is now Maintenance Supervisor at St. Lucie. Hallenbeck stated the Contract supervisor who reported the root opening incident was Lonnie Stansbury of United Engineers and Constructors (UE&C) and he can be located at UE&C in Philadelphia.

Hallenbeck could not recall who the welding engineer was who reported the DEH problem to him but it was one of the following: Lonnie Bates, Earl Squires, Ron Cable, Chuck i Crowser. Welder /Pipefitter, Bob Hight reported this to the welding engineer.

Maybe it was a open/close l case since we did not investigate all I&H during that I time period. Vorse called Uryc to see if he recalls the (

name. Uryc is checking.

12-16-94 TC from Linda Bouska: Left message, said Prevett told her I may need her assistance. Called her back on 12-19-94 and left her a message.

12-27-94: Received Smith's info, as requested.

1-5-95 VGS TC to Ernie Poarch 407-465-355- X4041, Voice mail answered, no message left. TC to Andy DeSoiza 4136:

Explained allegation and wanted to set up interviews. He asked me when we received the allegation and I told him and he asked if I had talked to Hallenbeck recently and I said yes and told him even though he settled with FPL and did not file a DOL concern, he is requesting NRC look into the matter. I informed DeSoiza I needed to talk to him and he said he would arrange an area for interviews and line up other witnesses as he knew who was involved and that there was a Speakout investigation. I gave him a list: Ernie Poarch, Herman Fagley, Jerry Kunkel, Dale Jacobs. DeSoiza said Speakout had forwarded the info. I requested also. Set interviews for 1-18-95 and 1-19 if needed. Informed him would have court reporter for the record.

Received Sneakout investigation results.
