SUBJECT: NRC INSPECTION REPORT 50-382/96-24 AND NOTICE OF VIOLATION
Dear Mr. Sellman:
An NRC inspection was conducted October **-25,1996, at your Waterford Steam Electnc
Station, Unit 3 reactor facility. The encic
.eport presents the scope and results of that
inspection.
During the inspection, two violations of NRC requirements were identified. The two
j
examples of the first violation indicate a less than rigorous resolution of identified
deficiencies in a timely manner. The second violation is of concern because your staff
failed to establish requirements to completely test the entire actuating system of
safety-related components.
These violations are cited in the enclosed Notice of Violation (Notice) and the
circumstances surrounding the violations are described in detail in the enclosed repurt.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your response. The NRC will
use your response, in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
One unresolved item was identified (pending resolution of Task Interface Agreement
96TIA017)concerning the closed safety function of certain containment isolation valves
that receive an open engineered safety feature signal and fail open on loss of air. This
matter is discussed in Section E2.1 of the enclosed inspection report.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure (s), and your response will be placed in the NRC Public Document Room
(PDR). To the extent possible, your response should not include any personal privacy,
proprietary, or safeguards information so that it can be placed in the PDR without
redaction.
