The subject deficiency was initially reported to NRC-0IE Inspector P. E.-Fredrickson on May 10, 1984 in accordance with 10 CFR 50.55(e) as NCR j-3069. This was followed by our first interim report dated June 4, 1984.

Subsequently, NCR 3457, which documents a similar condition, was initiated and was reported on with NCR 3069 in TVA's second interim report dated September _24', 1984. Two more items, NCR 3454 and NCR 3684, which document similar conditions, were initiated and TVA reported on all these NCRs simultaneously in the third interim report dated February 27, 1985. A related condition documented in NCR 4254 was reported on separately in a first interim l

report dated June 26, 1985 under CDR No. BLRD-50-438/85-18, BLRD-50-439/85-18. TVA intends to report on all of the aforementioned NCRs simultaneously. Enclosed is our fourth interim report. We expect to submit our next report on or about March 31, 1986. The date of November 22, 1985 for submittal of our next report on NCR 4254 as mentioned in our letter of June 25, 1985 is no longer valid.

FOURTH INTERIM REPORT

- Description of Deficiency Numerous deficiencies involving electrical conduit and conduit support f

installations which were initially inspected and accepted during the timeframe from 1979 to 1984 have been identified at Bellerante Nuclear Plant (BLN). 'Several nonconfccmance reports (NCRs) have been issued by TVA to document the subject deficiencies. NCRs 3684 and 4254 will be used to track these deficiencies and are identified below.

TVA has determined that the apparent cause of these deficiencies was a lack of knowledge regarding detailed design and construction requirements by personnel perfbraing electrical conduit and conduit support installation and inspection activities during the identified timeframe (1979 to 1984).

NCR BLN 3684 Due to the conditions identified by previously issued NCRs, TVA perfbemed a reinspection of 15 randomly selected electrical hanger installations which had been initially inspected and accepted during the 1979 to 1984 timeframe.

From this reinspection, 4 supports were identified with various deficiencies such as incorrect bolt sizes, inadequate welds, interferences, incorrect configuration, and incorrect mark number of support installed.

NCR BLN 4254 Numerous NCRs have been written to document deficiencies in conduit installations in the intake pumping station (IPS) at BLN. These installations were previously inspected and documented (prior to January 1, 1984) as acceptable. The NCRs identify deficiencies similar to those '

identified in NCR BLN 3684 above.

Interim Progress - NCRs 3684 and 4254' Due to the similarity of the problems associated with NCRs 3684 and 4254, TVA has initiated a reinspection program f6r all conduit and conduit support installations. All deficiencies identified during the reinsps ation program will be documented and corrected per applicable TVA procedures. Also, BLN Quality Control Procedure (QCP) 3 32 " Raceway Verification," has been revised to include the inspection criteria far conduit and supports. The walk-down inspections perfbrmed by QCP-3 32 will provide additional assurance that j

design requirements have been met for all conduit installations.

TVA will provide the next report to the NRC on this item on or about March 31,1986.
