Amendnent No. 03 CONDITIONS l

f A.

Each encapsulated plutonium source designed for the purpose of emitting neutron 13 or gama radiation shall be tested for leakage at intervals not to exceed six l

I (6) months.

In the absence of a certificate from a transfer indicating that a test has been made within six (6) months prior to the transfer, a sealed source l

received from another person shall not be put into use until tested.

l B.

The test shall be capable of detecting the pmsence of 0.005 microcurie of alpa pi contamination on the test sanple. The test sample shall be taken from the i

source or fran appropriate accessible surfaces of the device in which the sealed i

source is permanently or semipennanently mounted or stored. Records of leak (ql test results shall be kept in units of microcuries and maintained for inspection l

by the Comission.

i I

C.

If the test reveals the presence of 0.005 microcurie or more of removable alpha contamination, the licensee shall imediately withdraw the sealed source from f

use and shall cause it to be deco 6taminated and repaired by a person appropriately licensed to make such repairs or to be disposed of in accordance with the Comission regulations. Within five (5) days after detemining that l

k any source has leaked, the licensee shall file a report with the Material I

Licensing Branch, Division of Fuel Cycle and Material Safety U. S. Nuclear J

Regulatory Commission, Washington, D. C. 20555, describing the source, the test j

,i j

results, the extent of contamination, the apparent or suspected cause of source failure, and the corrective action taken. A copy of the report shall be sent to the U. S. Nuclear Regulatory Commission, Region I, Office of Inspection and ll Enforcenent, King of Prussia, Pennsylvania,19406.

l l

f D.

The periodic leak test requimd by this condition does not apply to sealed

>\\

sources that are stored and not being used. The sources excepted from shall be tested for leakage prior to any use or transfer to another person t

unless they have been leak tested withing six (G) months prior to the date of i

.use or transfer.

Except for plutonium contained in a nedical device designed for individual human 14.

.P aylication, no plutonium, regardless of form, shall be delivered to a carrier for sl11 ment by air transport or transported in an ain: raft by the licensee except in y)j 74

[q paccages the design of which the NRC has specifically approved for transport of 4

M plutonium by air.

jp Except as specifically provided othenvise by this license, the licensee shall possess l 15.

and use licensed material described in Items 6, 7, and 8 of this license in accordance ;
