' Arkansas Power & Light Company Docketi 50-368/85-24 Arkansas Nuclear One, Unit 2 License:
NPF-6 During.an NRC inspection conducted during the period September 1.30, 1985,
- violations of NRC requirements were identified.
The violations involved failure.to maintain an operating procedure in an up-to-date status and failure to properly maintain a pipe support.
In accordance with the " General Statement of Policy and Procedure'for NRC Enforcement Actions," 10 CFR Part-2, s Appendix C (1985), the violations are listed below:
- A.
Technical Specification 6.8.1. requires that, " Written procedures shall be.... maintained covering the... applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978."
Appendix A of Regulatory Guide 1.33 includes, " Procedures for startup, operation,' and shutdown.of... emergency core cooling systems."
Auxiliary System Operating Procedure 2104.40, " Low Pressure Injection
- System Operations," has been established in accordance with this
' Technical Specification requirement.
Contrary to the above Procedure 2104.40 has not been maintained in that the valve lineup sheets provided by the procedure did not contain all the valves installed in the low pressure safety injection system.
This is a Severity Level IV violation (Supplement I.0) (368/8524-01).
B.
Criterion-V of 10 CFR Part 50 requires that, " Activities affecting quality shall be prescribed by documented instructions, procedures, or-drawings, of a type appropriate to the circumstances and shall be accomplished-in accordance with these instructions, procedures, or drawings."
' Piping Isometric Drawing 2HCC-32-3 shows the location of pipe support 2HCC-32-H16 on;11ne'2HCC-32-3.
This is part of the drain line-from:the reactor drain tank to' the waste management system.
Piping and Instrumentation Drawing M-2214' indicated that this section of line had been: upgraded to seismic class I.
Drawing 2HCC-32-H16 showed the configuration of pipe support'2HCC-32-H16.

~ Contrary to the above, on September 10, 1985, the NRC inspector observed that a chain fall was attached to pipe' support 2HCC-32-H16,. applying an unanalyzed stress on the. support in a horizontal direction.

This is a Severity Leve1 IV violation (Supplement I.D) (368/8524-02).
. Pursuant.the provisions of 10 CFR Part 2.201, Arkansas Power and Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken'and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and
- (4) the date when full compliance will be achieved. Where' good cause is shown, consideration will be given to extending-the response time.
