Subject: Inspection No. 50-293/85-17
This refers to the routine safety inspection conducted by Mr. J. Johnson and Dr.
M. McBride of this office on June 13 - July 15, 1985 at the Pilgrim Nuclear Power
Station, Plymouth, Massachusetts. The inspection consistea of document reviews,
interviews, and observation of activities. The findings were discussed with Mr.
P. Mastrangelo of your staff.
Based on the results of this inspection, it appears that certain of your activities
related to establishment and adherence to surveillance test procedures and to ad-
herence to health physics procedures were not conducted in full compliance with
\ NRC requirements, as set forth in the Notice of Violation, enclosed herewith as
Appendix A. The violations have been categorized by severity level in accordance
,/ with the revised NRC Enforcement Policy (10 CFR 2, Appendix C) published in the
Federal Register Notice (49 FR 8583) dated March 8, 1984. You are required to re-
spond to this notice and in preparing your response, you should follow the in-
structions in Appendix A.
In addition, it appears that your Inservice Testing (IST) Program did not fully

implement commitments contained in your submittals to the NRC, as set forth in the
Notice of Deviation, enclosed herewith as Appendix B. You are requested to respond
to this notice and in preparing your response, you should follow the instructions
in Appendix B.
The responses directed by this letter and the accompanying Notices are not subject
to the clearance procedures of the Office of Management and Budget as required by
the Paperwork Reduction Act of 1980, PL 96-511.
