( Occumentation of Possible Allegations at Grand Gulf C. A. Julian, Section Chief, PRP, Region II On 10/19-21/1983 I participated with Bruce wilson and other regional based inspectors in a training assessment at drand Gulf. The stated purpose was to assure that the presently licensed operators are adequately trained and to see that old problems in the training department that led to false applications being submitted to the NRC were corrected.

  • 4 Our overall findings were negative in that we identified individual operators with training problems and we found that applications for NRC SRO exams of September 26, 1983 were rushed. I did not review the applications to see if they were false but the training done to support the applications certainly did not

( 1eet the intent af their FSAR commitment.

t During the inspection we reviewed many documents but were particularly alarmed at the qualification cards for four instant SRO's who took the September 26 NRC exam.

The qual card' is a signoff check sheet to be initialed by a qualified person to certify that a candidate has completed a given facet of his training, such as a walkdown and oral exam on a particular reactor system. These four qual cards contained only signatures of Quadrex training instructors, who are not NRC licensed and far too many systems were signed off in a short time.

There were over 100 signoffs done on each man in one day which means they could not possibly have done a good review of the man's knowledge in all areas.

q i j On 10/20/83 Mr. Wilson and I interviewed the Quadrex people who signed the qual cards. They asked if we wanted to talk separately or in a group and I said group

discussion was OK. We asked how they could accomplish that many signoffs in one i

day. They said they did the best they could in the time they had allotted. We asked if they thought these rushed methods were good training practice and they said they certainly were not. In fact they said it was " crazy" and a " complete farce". I asked who told them to do this and they said MP&L. I asked if they protested these methods and they said they had but were told to do it anyway.

One Quadrex person said that when told of the schedule by the MP&L training l

t supervisor he strongly objected. Two days later he was called to a meeting ,

I chaired by the MP&L plant manager in which he was told to meet the established schedule or else and that there would be no further discussion. The other Quadrex instructors also-stated that this was their understanding of the sequence of events. They gave me a copy of the schedule they were told to meet and a copy I

b Consequently the four Quadrex instructors went through the qual cards with the four candidates as a group, and completed all remaining signatures in three days.

(8/30-9/2/83) All this work was done in the simulator and no actual plant walk throughs were done. These methods are, in my opinion, contrary to the intent of the qual card program, however, Grand Gulf's training. procedures are vague enough to allow one to interpret this to be allowed.

The Quadrex people assured us that they would not have signed the qual cards if

the individuals were totally unsafe and incompetent. I asked if they thought the four'were ready for the NRC exam and they said not all of them. I asked if'they y .-

recommended that the weak candidates be withdrawn and they said yes they did, but e

ao

3 MP&L didn't pay any attention. I asked if MP&L had put up candidates before for (v)

NRC exams, despite Quadrex recommending against it and they said yes. One individual stated that he thought MP&L would put up anyone regardless of previous training because they need the licensees. 10 CFR 55.10 requires that MP&L certify to the NRC in the application for exams that the candidates have been adequately trained.

The Quadrex supervisor ended our meeting by asking that we not reveal' the source of this information or else he would "be off this site before sundown." During subsequent exit interviews with MP&L we avoided using names or specific details which would reveal our source.

I have documented these facts of our ir.spection as a potential allegation, as y ,) these activities appear to relate to poten.tial material false statements in applications for NRC exams, MP&L management directing contractors and employees to

. cut corners on operator training to meet their startup schedules and potential of intent to deceive the NRC. At MP&L's request, the exams for the four candidates plus two other upgrades were moved up to September 26 by postponing exams for another licensee. During a management meeting on August 12, 1983 in Region II the agreement was made to advance the exam date. MP&L representatives assured Region II that the candidates would complete their training in an acceptable manner prior to September 26. It now appears that the training was so rushed it could not have been adequate. Two of the four candidates failed their exam.
