REQUEST FOR INVESTIGATIVE ASSISTANCE: GRAND GULF NUCLEAR PLANT (CASE NO. RII-84-A-0023)

This refers to the meeting between Jeff Lankford and Larry Robinson on February 9,1984, relative to the possible falsification of training records for mechanical maintenance personnel at Grand Gulf. The enclosed Allegation Report provides the details of this allegation. Please note that the Report contains the identity of the individual who provided the information and every effort must be made to preclude the inadvertent disclosure of that information, inasmuch as he has requested confidentiality.

We recuest that you expand the scope of the on going 01 investigation (2-83-037) involving operator training at Grand Gulf to include the information provided herein. We will provide any technical assistance that you may need at your request. If information of any significance is developed through your investi-gation of this matter, please inform me immediately. If you have any questions n)

1) Select the necessary rigging to use with a hoist to make a vertical lift. -
2) Perform a vertical lift of 2-3 feet, then lower it.
3) Safely remove a load from a truck and transport it, using a forklift.
4) Safely atack and unstack pallets, using a forklif t.
5) Disassenble a coupling in pre-paration for shaf t alignment.
6) Inspect the parts of a dis-assembled coupling for wear or damage.
7) Detect angular misalignment using feeler gauges.
8) Detect parallel misalignment using a dial indicator.
9) Reassenble a coupling following i alignment.

l have information that I wish to provice in confidence to the U. 5. Nuclear Kegulatory Commission (NRC). I request an express pledge of confidentiality as a condition of providing this infortnation to the ARC. I will not provide this information voluntarily to the NRC without such confidentiality being extended to me.

It is my understanding, consistent with its legal obligations, the NRC, by agreeing to this confidentiality, will adhere to the following conditions:

(1 ) The NRC will not identify me by narne or personal identifier in any NRC initiated document, conversation, or cor:Tnunication released to the public which relates directly to the information provided by me. I understand the terin "public release" to encomoess any distribution outside of the NRC witn the exception of other public agencies which may require this infort.ation in

'futherance of their responsibilities.under law or public trust.

(2 ) The NRC will disclose my identity within the NRC only to the extent reouired for the conduct of NRC related activities.

(3) During the course of the inquiry or investigation the NRC will also make every effort consistent with the investigative needs of the Commission to avcid actions which would clearly be expected to result in the disclosure of my identity to persons subscouently contacted by the NRC. At a later stage I understand that even thouer the NRC will make every reasonable ef fort to protect my identity, my identification could be corspelled by orders or subcoenas issued by courts of d l'aw, hearing boards, or similar legal entities. In such ceses, the basis for granting this promise of confidentiality and any other relevant facts will be cc miunicated to the authority ordering the disclosure in an effort to maintain my con ficenti6;i ty. If this effort proves unsuccets ful, a ree es entative o' the NRC will attempt to inform me of any sucn action cefore oisciosing my idenu ty.

I also understand that the NRC will consider me to have waived my right to confidentiality if I take any action that may be reascnably exoected to disclose my identity. I further understand that the NRC will consicer me to have waived my rights to confidentiality if I provide (or have previously provided) information to any other party that contradicts the information that I provided to the NRC or if circumstances indicate that I am intentionally oroviding false information to the NRC.
