BELLEFCNTE NUCLEAR PLANT INIT 1 - EXCESSIVE WIRING IEFICIENCIES IN BABCOCK AND WILCOX SUPPLIED CABINETS - BLRD-50-438/84 SECOND INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector P. E. Fredrickson on November 14, 1984 in accordance with 10 TR 50.55(e) as NCR 3675.
Our first interim report was sutinitted on December 12, 198't.
Enclosed is our second interim report. We expect to subnit our next report on er about July 3, 1985. We consider 10 TR Part 21 applicable to this deficiency.

SECOND IN1ERIM REPORT Description of Deficiency The emergency safegtards features actuation system (ESFAS) control cabinets for Bellefonte Nuclear Plant (BLN) were supplied to TVA by Babcock and Wilcox (MW) on the BLN NSSS contract. The initial site iropection of these cabinets resulted in at least 68 separate nonconformance reports (NGs) being issued for wiring and drawing deficiencies. A large ntaber of the NCRs identify installed wiring which cbes not conform to B&W drawings with mapect to color codes, wim size, or configuration. Several NCRs identify breakers which are of a higher rating than the MW daawings show. While rene of these 68 NGs represent a significant conditicu adverse to quality on an individual basis, they collectively indicate a significant breakdown in MW's quality assuranoe/ quality control (QA/QC) program for BLN.
Interim Progress B&W has been notified of the subject individual NCRs and the need to evaluate vendor QA/QC programs to prevent recurrence of equipnent deficiencies. MW is proceeding with corrective action to maolve each individual NCR and has scheduled an audit fcr the purpose of determining the root cause of the wiring deficiencies.
