This letter is in response to your letter dated September 16, 1980 which contained a Notice of Violation requiring a response from me.

The Notice of Violation was related to the performance of a portion of a safety related i

Surveillance Procedure QO-2 which was not conducted in full compliance with the requirements of my Senior Operator License No. SOP 3160-1.

Because my signature appeared on the QO-2 Surveillance Procedure of f

August 19, 1980 as the authorizing Shift Supervisor, I must accept full i

responsibility for my actions and the events which followed and caused the

5 During the past eleven years at this plant, I had, performed this Surveillance Procedure as a Licensed Operator many times and in the last three years as a Shift Supervisor I had authorized the performance of the QO-2 Surveillance Procedure during plant power operation previous to plant shutdown in September 1979 2

License Event Report 80-005 reported an apparent conflict between the surveillance requirements and equipment operability requirements of the

i-Procedure QO-2 were changed to require that the plant be in a cold shutdown or refueling shutdown conditions prior to performance of the Surveillance Procedure.

In issuing two Surveillance Procedures on the morning of August 19, 1980 I read through and checked the first procedure and then noting that the second procedure title was QO-2 and because of my familiarity with this Surveillance Procedure in the past, I authorized performance of the procedure after a quick perfunctory check and failed to notice that the plant status requirement had been changed.'
