APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458 River Bend Station Permit: CPPR-145 During an NRC inspection conducted April 3-8, 1985, a violation of NRC require-ments was identified. The violation involved an inadequate procedure. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:

Inadequate Test Procedure 10 CFR Part 50, Appendix B, Criterion V states that procedures, drawings, and instructions shall be used to control activities affecting quality related activities.

Contrary to the above, Preoperational Procedure 1-PT-57-1 failed to identify six differential pressure instruments and one motor operated l

valve required for the valve lineup to conduct the Intergrated Leak Test.

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) the reason for the violation if admitted, (2) the corrective steps which hata been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
