Inspection on May 30, 1985 by a representative of the State of Washington
Department of Social and Health Services
Philadelphia Electric Company was the originator of a waste shipment which was
inspected at the U. S. Ecology, Incorporated burial site.
Results: One violat' ion was identified (failure to provide a strong, tight
package).

DETAILS
On May 20, 1985, the licensee dispatched a shipment of radioactive waste
material (licensee's shipment No. 103-85) to the U. S. Ecology, Incorporated,
low-level radioactive waste disposal site near Richland, Washington. The
shipment included a 55 gallon drum (licensee's No. S-146-85) containing
solidified oils with low specific activity (LSA) licensed radioactive
materials.
The shipment was transported in an exclusive-use vehicle.
On May 30, 1985, a representative of the State of Washington, Department of
Social and Health Services inspected the shipment. The inspection consisted
of a review of shipping papers, placarding, marking and labeling, radiation
measurements and examination of the packages and the tractor-trailer. A copy
of the report prepared by the representative is attached.
Visual examination of drum No. S-146-85 by the state representative identified
a hole in the drum. A representative of the licensee confirmed the presence
of a " puncture hole" above the lifting ring of the drum following his examin-
ation of the drum at the U. S. Ecology, Incorporated site.
10 CFR 71.5 requires each licensee who delivers licensed material to a carrier
for transport to comply with the applicable requirements appropriate to the
mode of transport in 49 CFR 170 through 189.
49 CFR 173.425(b)(1) requires
LSA materials be packaged in strong, tight containers when transported as
exclusive-use.
Failure to package low specific activity solidified oils in
a strong, tight container constitutes a violation of a 10 CFR 71.5,
50-277/85-27-01; 50-278/85-25-01.
The Support Health Physicist was informed of this violation during a telephone
conversation with Mr. Bicehouse on June 19, 1985. No information exempt from
disclosure under 17'CFR 2.790 was discussed in this report.

Summary:
Inspection of May 30, 1985 (Report No. 85-02)
As a part of the inspections conducted by the State of Washington at the
Richland Burial Site, a shipment of low level waste from Philadelphia Electric
Company, Philadelphia, Pennsylvania was inspected on May 30, 1985.
The
results of that inspection which were received at the NRC Region V office on
June 10, 1985 are detailed in this report.
Results:
One apparent violation of NRC/ DOT regulations was identified.
It
appears that the NRC Region I office, the Department of Transportation, and
the State of Washington are the appropriate organizations to take enforcement
actions.

Background
As part of the low-level waste inspections conducted by the State of
Washington inspector at the Richland Burial Site, one shipment was found to be
in violation of NRC/ DOT regulations. The shipment was received on May 30,
1985. One apparent violation was identified.
Pursuant to the violation, the
State of Washington banned the NRC license from using the burial site and
terminated Permit No. 5625 effective May 31, 1985.
