,e.,*.. w.a i2 i2 DESCRIPflON OF AMENDMINT/MODIFICAnON The purpose of this modification is to change the place of performance under the contrac1 in accordance with the " changes" clause and to increase the amount obligated by the Government.

This modification results in the following changes to the contract:

1.

Under SECTION E ADD the following item:

"3.

The Contractor shall furnish Quantity _ Unit Unit Price Amount office space as required for the counseling sessions be-4 mo.

ending September 16,1980, at a fixed rate of $200 per month."

The total amount obligated under this contract is increased by $900.00 from

$16,182.00 to $17,082.00.

Your attention is directed to Clause No. 2 to the General Provisions of the Contract entitled " Changes".

Please note that any claim by the Contractor for ad-justment under this clause must be asserted within 30 days from the date of receipt by the Contractor of the modification of change.
