SUBJECT: NRC INSPECTION REPORT 50-361/96-15; 50-362/96-15 AND NOTICE OF

VIOLATION

Dear Mr. Ray:

An NRC inspection was conducted October 20 through November 30,1996, at your San

Onofre Nuclear Generating Station, Units 2 and 3 reactor facilities. The enclosed report

'

presents the scope and results of that inspection.

During the 6 week period covered by this inspection, your conduct of activities at the San

j

Onofre facilities was generally characterized by safety-conscious operation, sound

l

engineering and maintenance practices, and careful radiological work controls.

i

One of your activities was in violation of NRC requirements. This violation, involving an

inspector's identification of a mispositioned component, is cited in the enclosed Notice of

I

Violation (Notice) and the circumstances surrounding the violation are described in detailin

'

the enclosed report. Please note that you are required to respond to this letter and should

,

follow the instructions specified in the enclosed Notice when preparing your response. As

'

discussed in our telephone conservation, we note the recent situations involving

l

,

mispositioned valves or components which have been identified by your staff and the NRC.

I

in addition, your Nuclear Oversight Division recently performed an assessment which

,

documented other such situations in 1996. Therefore, in addition to responding to the

specific violation cited, we request that you provide in your response a description of your

'

assE,ssment and corrective actions to broadly address your planned and implemented

I

corrective actions to preclude valve and component mispositioning events in the future.

The NRC will use your response, in part, to determine whether further enforcement action

'

is necessary to ensure compliance with regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's ' Rules of Practice," a copy of this letter,

its enclosures, and your response will be placed in the NRC Public Document Room (PDR).

To the extent possible, your response should not include any personal privacy, proprietary,
