NRC INTEGRATED INSPECTION REPORT 50 327/96 09 AND 50-328/96-09

NOTICE OF VIOLATION

Dear Mr. Kingsley:

On September 14, 1996, the NRC completed an inspection at your Sequoyah

facility. The enclosed report presents the results of that inspection.

During the period covered by this inspection period, your conduct of

activities at the Sequoyah facility was generally characterized by acceptable

operations, engineering, maintenance practices, and radiological work

{

controls. We are concerned, however, about a violation of NRC requirements

that was identified. Corrective actions, associated with the emergency diesel

generator (EDG) starting air compressor pre;sure control switch failures, were

considered to be untimely and inadequate.

In addition, the root cause

determinations for the 1995 and initial 1996 problems were considered to be

weak in that they failed to identify the defective EDG compressor pressure

switches.

We are also concerned that repetitive equipment problems continued to cause

added operator burden ard impacted maintenance resources, operation's

switching and tagging resources, and da) to day operations.

Problems with

plant material condition indicate weaknesses in maintenance and engineering

,

support.

This violation is cited in the enclosed Notice of Violation (Notice), and the

circumstances surrounding the violation are described in detail in the

enclosed report.

Please note that you are required to respond to this letter

and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to deter-

mine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
