~Y Office of Nuclear Reactor Regulation Items of Interest Week Ending February 12, 1993 Seabrook Station. Unit 1 On February 5, 1993, the Securities and Exchange Commission (SEC) approved a plan for EUA Power Corporation (EUAP) to redeem all its common and preferred stock from Eastern Utility Associates (EUA).
EUAP, organized as a wholly-owned subsidiary of EUA, holds a 12.13% ownership in the Seabrook Station.
EUAP filed for protection under Chapter 11 of the Bankruptcy Code in February 1992.
The planned redemption of stock will change the status of EUAP from that of a subsidiary of a holding company to that of a stand-alone entity, thus eliminating the statutory requirement for the SEC (under the Public Utility Holding Company Act of 1935) to review and ap3 rove the reorganization plan prior to the Bankruptcy Court's approval of t1at same plan.
The change of status eliminates the lengthy delay of duplicate review and approval of.the plan, and clears the way for a scheduled Bankruptcy Court hearing on March 5, 199'.
Following Bankruptcy Court approval of a reorganization plan, any associated change in ownership will be reviewed by the NRC under 10 CFR 50.80.
The station is operating at 100% power with no operational problems.
Yankee Rowl On Wednesday, february 10, 1993, the staff met with Yankee Atomic personnel.
The licensee requested the meeting in order to update the staff on the status of planning for the decommissioning of the plant and the status of the preparation of the decommissioning plan.
Yankee hopes to submit the plan by October 1993; thus beating the February 1994 due date.
The Yankee Atomic Electric Company board of directors will meet in late February and make a decision on removal of the four steam generators, RV internals and pressurizer.
The licensee discussed their plans to retain and if necessary replace key personnel who may leave Yankee employment, thereby maintaining Technical Soecification staffing requirements.
Yankee will hold a public meeting on Thursday evening, February 18, 1993, at the Visitors-Conter to present-their decommissioning program. They expect 150 to 200 attendees including local officials. The Senior Resident Inspector plans to observe the meeting.
BWR Water level Instrumentation Ashok Thadani, Timothy Collins, and Amy C_ubbage attended & public meeting on February 3, 1993, in Plymouth, Mass. The purpose of the meeting was to update local officials and area residents. regarding the BWR water level instrument issue as it pertains to the Pilgrim station. The staff provided a brief~
update on the status of current and plann_ed BWROG activities to resolve this issue, and also an update of the status of the water level instrumentation at FEBRUARY 12, 1993 ENCLOSURE.A

Public Meetina with NUMARC on the Maintenance Rule 1molementation Guideling Richard Correia, Chuck Petrone, Tom Foley (NRR), and Owen Rothberg (RES) participated in a public meeting with representatives from NUMARC on February 9, 1993. The meeting was held (at OWFN) to discuss the addition of examples to the NUMARC maintenance rule implementation guideline which address emergency diesel general (EDG) reliability and availability.
Based on the i
discussions, NUMARC will prepare a final written version of these additions i
and submit them to the NRC around February 23, 1993. NUMARC also stated that they intend to issue Revision 3 to their guideline by mid-March 1993.
The.
revision would include the proposed EDG additions and results from the recently completed NUMARC/ industry verification and validation process for the i
guideline.
Meetina With Westinohouse To Obtain Additional Information On AP600 Valves On Friday, February 5, 1993, the NRC staff met with Westinghouse at the J
Westinghouse Energy Center in Monroeville, Pennsylvania, to obtain information needed to assess the reliability of valves in the AP600 design.
Westinghouse provided a brief overview of the passive safety systems used in the AP600. They discussed functional requirements, valve (and actuator) characteristics, and proposed valve testing requirements for check valves, motor operated valves, air operated valves and associated solenoid valves, and squib valves that the staff had identified as being " critical" valves for its passive reactor valve study. The staff expressed-specific concerns where it appeared that the AF600 design may not be able to accommodate Code required testing and that the ability to test valves to verify capability under design basis conditions may be severely limited in some cases.
For example, the in-series core makeup tank discharge check valves may not be able to-be tested in the reverse flow closure direction. The design for testing of the ADS' Stage 1, 2, and 3 MOVs will only accommodate very low flow testing.
The staff also expressed concerns with the lack of a basis or plans to establish a basis for the reliability numbers thi.t Westinghouse is relying on in its probability calculations.
Most of the information provided was preliminary.
For each valve Westinghouse provided a draft valve specification in the form of." Control Valve Data Sheets." The staff indicated to Westinghouse that it would like to get more detailed Control Valve Data Sheets for each of the " critical" valves. The staff indicated several areas where more specific information in the valve specification is needed before valve reliability estimates can-be assessed.
Reaion I Office Review During the week of February 8, 1993, the Operator Licensing Branch (HOLB) conducted an annual office review of the operator licensing function in Region I.

Inspection On February 8, 1993, an Augmented Inspection Team (AIT) was dispatched to Siemens Power Corporation in response to an inadvertent accumulation of approximately 120 kilograms of low enriched uranium dioxide. The uranium spilled from a feed hopper onto the floor of a lexan enclosure that
- i encompasses the feed hopper. The accumulation was caused when an automatic engineered control failed.
The AIT charter includes investigation of the root cause of the failure.
Seouovah Fuels Corporation On February 10, 1993, a public meeting was held between representatives-of Sequoyah fuels Corporation (SFC) and the Nuclear Regulatory Commission to-discuss SFC's plans for future operations and the related licensing impli-cations.
SFC announced its decision to terminate all licensed activities.
They will proceed with decommissioning the UF6 facility. After completion of the present contract (July 31,1993), they will proceed with decommissioning the. depleted UF4 facility.
On February 16, 1993 SFC intends to file a notification pursuant to 10 CFR 40.42(b) to terminate activities.under License-SUB-1010 that will be effective on July 31, 1993. The notification will include a Preliminary Plan for Completion of Decommissioning, which will include SFC's plan for assuring the availability of adequate funds, cost estimates, and schedules.
The current cost estimate is $21.2 million over a.
10-year period.
SFC also plans to meet with the parties and file a motion with the Atomic Safety and Licensing Board to withdraw the licensc renewal application as of July 31, 1993, and terminate the proceeding.
International Safeauards Activities On February 10, 1993 International Safeguards representatives participated in the interagency Technical Support Coordinating Committee (TSCC) meeting.
This group is-responsible for planning, developing and overseeing a U.S.
coordinated program in support of the International Atomic Energy Agency (IAEA) safeguards. During the meeting, funding was allocated for a Nuclear Regulatory Commission expert to act as a consultabt to the IAEA this spring in the area of evaluation of IAEA safeguards effectiveness.
The TSCC also reviewed the FY93 funding and proposed projects as well as other issues including extension of several cost-free experts, establishment of a system for management control of contracts, and review of future international meetings related to technical support from other IAEA member states.- A~
coordination meeting of the TSCC with other member states is planned for late March and a review meeting with the IAEA is planned-for late April at Brookhaven National Laboratory.

2 audit of the Civilian Radioactive Waste Manaaement System ManaaNent and Ooeratina Contractor (M&O)
On February 1-5, 1993, quality assurance (QA) staff of the Division of High-Level Waste Management (HLWM) and the Center for Nuclear Waste Regulatory Analyses (CNWRA) observed a compliance-based QA audit of the M&O at the office of TRW Environmental Safety Systems, Inc., Vienna, Virginia. This was the first audit of the M&O observed by the HLWM and CNWRA staff to determine if it meets the Office of Civilian Radioactive Waste Management requirements.
The HLWM and CNWRA observers determined that the audit was effective and agreed with the audit team thet im)1ementation of the M&O QA program was marginally effective overall for ne QA programmatic elements that were audited. As a result of the audit, nine preliminary Corrective Action Requests (CARS) were issued by the audit team. The adverse conditions identified in the CARS are not significant in terms of the overall M&O QA program.
Audit of the Technical and Malulaement Support Service Contractor (T& MSS 1 Division of High-level Waste Management (HLWM) and Center for Nuclear Waste Regulatory Analyses (CNWRA) quality assurance (QA) and technical staff observed a U.S. Department of Energy, Office of Civilian Radioactive Waste Management, Yucca Mountain Quality Assurance Division (YMQAD), QA audit of the 1& MSS in Las Vegas, Nevada and at the Nevada Test Site during February 1-4, 1993. The audit evaluated the adequacy and effectiveness of the T& MSS QA program in nine programmatic and one technical area. The State of Nevada did not observe this audit.
The HLWM and CNWRA observers determined that the YMQAD audit was effective, and agreed with the audit team that the T& MSS QA program has been implemented effectively in the areas audited. Three procedural deficiencies were corrected during the audit, and no Corrective Action Requests were issued by the audit team.
Cateaory 1 Dam Insoection at Farlev Nuclear Plant The first Category 1 (nuclear safety-related) dam associated with a power reactor facility was inspected at the Farley Nuclear Plant on February 2-3, 1993, with the technical assistance of the personnel from the Federal Energy Regulatory Commission (FERC) under the Memorandum of Agreement and the Interagency Agreement with the Nuclear Regulatory Commission.
Personnel from the Office of Nuclear Reactor Regulation, Nuclear Material Safety and Safeguards and Region 11 also participated in the inspection effort. Only minor maintenance items were found in-need of attention along with a need to check instrumentation functional capability at two locations within the observation network.
The dam facility will continue to be classified under the Federal Guidelines for Dam Safety as a low hazard dam (no loss of life would be expected from dam failure).

Petition for Rulemakino 50-56 On August 16, 1993 Richard P. Grill submitted a petition for rulemaking
-(PRM 50-56). The petitioner requested that the NRC amend 10 CFR 50 to add lightning induced and other electrical transients to the required list of phenomena that nuclear power plants be designed to safety withstand. The petition has been denied because it was concluded that the design and construction of existing nuclear plants adequately protect plant electrical systems from the effect of electrical transients, and there is no evidence at this time that additional generic regulatory action is required.
The denial of the petition for rulemaking was published in the Federal Register on February 9, 1993.
Reactor Safety Course (R-800)
The first (dry-run) presentation of the one-week Reactor Safety Course (R-800) was held on February 1-5, 1993, at the Technical Training Center in-Chattanooga, TN. The attendees represented all of the regions as well as Headquarters offices. Topics covered were an historical overview of NRC policies and regulations, accident sequences, beyond-design-basis accidents, accident progression in the reactor vessel and the containment, radiological releases and consequences, and emergency response, in response to an AE0D user request to REe the course content was developed by staff from RES, AE00, Sandia Nationu Laboratories (SNL) and the University of New Mexico (UNM).
It was taught by SNL and UNM staff. The intended audience includes interns, inspectors and other NRC staff who are concerncd with reactor safety.
The dates for additional course presentations are now being established.
Costs of Radioloaical Criteria for Decommissionina On February 10, 1993, staff from RES and NHSS met with representatives from EPA, DOE, and D0D to identify potential sources of data on incremental costs of remediation, given current criteria, that could ' result from various approaches to setting radiological criteria for decommissioning.
Such data is expected to be useful in the preparation of the draft Generic Environmental Impact Statement and the Regulatory Analysis for the Enhanced Participatory Rulemaking on decommissioning. The meeting provided information on a number of areas that will be followed up by the RES staff.

i Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending February 12, 1993 Diaanostic Evaluation and incident Investiaation Branch (DEllB) l On February 8, a Commission briefing was held to discuss the findings and conclusions resulting from the Incident Investigation Team (llT) of a loss of an iridium-192 source and therapy misadministration at Indiana Regional Cancer Center located in Indiana, Pennsylvania on November 16, 1992. A highly radioactive iridium-192 source became detached from a source wire from an Omnitron 2000 High Dose Rate afterloader device and remained in the patient.
l The patient subsequently died 5 days later.
Dr. Carl Paperiello, Team Leader for the !!T, made the presentation to the Commissioners, with assistance from
!!T members.
Supplemental remarks were given by the Indiana County Coroner and the Food and Drug Administration.
This !!T report was made publicly available at the meeting.
At the direction of the EDO, an 111 was formed to investigate the February 7 unauthorized forced entry into the protected area at the Three Mile Island Unit 1 (TMI-1) Nuclear Generating Station. The team consists of Samuel Collins, IIT Leader, and Lee Miller, James Creed, Emilio Garcia, Charles Gaskin, William Hutchinson, Peter Moeller, Michael Warren, Gail Barkdoll, Edythe Becker, and Cherie Siegel as team members, consultant and administrative support. Observers included Rae Scott (IG) and representatives for the State of Pennsylvania.
Incident Resnonse Branch 9
On February 7, the Agency entered the Standby Mode in response to a licensee declared Site Area Emergency (SAE) at THI-1.
The Regional incident Response Center and the Headquarters Operations Center was staffed appropriately.
The SAE was declared when an intruder broke through the outer security fence with a stationwagon and crashed through a cargo access door to the turbine building. The licensee-de-escalated from the SAE--after the intruder was -
apprehended and the necessary precautionary measures had been completed.
On February 9, IRB staff conducted a tour and briefing of the Operations Center for two Swedish Nuclear Power Inspectorate.(SK!) representatives.
IRB staffed conducted further discussions on February 10 with the Swedish representatives on "NRC Actions in Connection with Incidents and Accidents."
On February 10, the IRB staff participated-in a meeting of the Annex Planning Leaders for the Federal Response Plan (FRP) to discuss possible revisions to the FRP. These revisions would address problems that surfaced during the Federal Response to Hurricane Andrew.

Office of Administration Items of Interest Week Ending February.1?, 1993 Unfettered Access to Protected and Vital Areas of Nuclear Facilities Modifications to incorporate a clause in five contracts requiring unescorted access to the protected and vital areas of nuclear facilities were mailed to the responsible contractors on February 11, 1993.
The clause was developed at NRR's request, and concurred in by the Division of Security and the Office of the General Counsel.
TWFN Auditorium A committee has been formed to develop policy and procedures for the public use of the Two l'hite Flint North auditorium. The committee consists of representatives from the Chairman's office, EDO, OGC, OPA, CONS and ADM.
The auditorium has a planned seating capacity of 300, and as required by the Montgomery County Planning Board, must be made available for public use.
Egicarch Grant Ano11 cations Seventy-four applications have been received in response to NRC's announcement of availability of FY 93 funds for financial assistance. Applications were solicited for the expansion, exchange and transfer of knowledge, ideas and concepts directed toward the NRC safety research program.
The applications will undergo technical evaluation and review by a financial assistance board before final determination on funding are made.
Licenses and Radiation Safety Reouirements for Irrad_i_ators A final rule entitled " Licenses and Radiation Safety Requirements for Irradiators" was published in the Federal Register on February 9, 1993 (58 FR 7715). The final rule specified radiation safety requirements and licensing requirements for the use of licensed radioactive materials in irradiators.
The final rule becomes effective July 1,1993.
Richard P. Grill: Denial of Petition for Rulemakina A denial of a petition for rulemaking submitted by Richard P. Grill (PRM 58) was published in the Federal Register on February 9, 1992 (58 FR 7757).
The petitioner requested that the NRC amend 10 CFR Part 50 and issue new regulations, as necessary, to_ add lightning induced and other electrical transients to the list of phenomena that nuclear power plants and other nuclear facilities must be designed to withstand safety. Office of Information Resources Management items of Interest Week Ending February 12, 1993 Mgitimedia Suppgri lhe Autom:ted Graphics Section (AGS) is developing new creative imaging technologies in initiating computer graphics designs for the Division of Security.
The AGS is preparing NRC Security: "You Are The Key," which was originally prepared on a 3/4" video tape, for a computer-related imagery of motion picture and sound. This computer-based, multimedia presentation will be used as a training device as well as instituting new trends in computer technology at the NRC.
Information Handlina Servicel The information and Records Management Branch conducted a survey of the Information Handling Services' microfiche subscription service of the updated final Safety Analysis Report arid certain codes and standards to determine the continued need for the service. As a result of the survey, subscriptions were canceled resulting in annual savings of $33,360.

I Office of Personnel ltems of Interest Week Ending February 12, 1993 Orientation Held for New SES and SLS Employees On February 10, the Executive and Operations Support Programs staff conducted a day-long orientation session for new Senior Executive Service (SES) and Senior level System (SLS) employees.
The four SES participants (Susan Shankman, Region 1; Ken Brockman, AE00; Bob Gallo and Jack Strosnider, NRR) and SLS participant (John Knubel, ED0) met with the Chairman, the EDO and his executive staff, the executive staffs of NRR, HMSS, and RES, as well as the directors of IP, CA, and PA for informal discussions.
Sexual Harassment Prevention Trainino Conducted in Reaion IV Dennis Dambly and Carl Mohrwinkel 0GC, and Mary Louise Roe, OP, conducted Sexual Harassment Prevention Training for Region IV employees during the week of February 1.
Four half-day sessions were held in Arlington, Texas, and one half day session was conducted at the Uranium Recovery Field Office (URFO) in Denver, Colorado.

4 Office of Small and Disadvantaged Business Utilization and Civil Rights items of Interest Week Ending February 12, 1993 On February 8,1993, SBCR, OP and OC staff met with representatives from Oak Ridge Institute for Science ar d Education (0 RISE) to establish an implementation plan and schedule for the NRC/HBCU Research Participation Program. The program supports research education and training efforts benefiting HBCUs and the federal government, includes assigning HBCU faculty and students to participate in research and engineering activities it federal facilities, and promotes partnerships with HBCUs for grants, contracting, and recruitment purposes.
On February 9,1993, the Minority Business Development Agency Working Group met at NASA to discuss its minority business program agerda for 1993. Harris Coleman represented SBCR in a discussion of agencies' participation in Minority Enterprise Development Week scheduled for October.

Office of Enforcement Items of Interest Week Ending February 12, 1993 Sionificant Enforcement Actions 1.
A Notice of Violation and Proposed imposition of Civil Penalty in the amount of $37,500 was issued on February 8, 1993 to the Indiana Hichigan Power Company (DC Cook). This action was based on a violation that resulted from one of the unit's emergency diesel generators being inoperable for a period of time in excess of that allowed by the Technical Specifications. The emergency diesel generator was rendered inoperable by an insufficient amount of lube oil.
Licensee 3ersonnel had earlier recognized the oil leak that eventually led to t1e operability problem but for a numtar of reasons failed to fix the leak or take compensatory actions to offset its effect. This violation has been classified at Severity Level 111.
The base civil penalty of
$50,000 has been mitigated 25 percent after concluding mitigation of 25 percent was appropriate for the licensee's identification of this self-disclosing event, mitigation of 50 percent for corrective actions, and 100 escalation for the prior opportunities to prevent the violations.

A Notice of Violation and Proposed imposition of Civil Penalty in the amount of $225,0C0 was issued on February 10, 1993, to Carolina Power &
Light Company (Brunswick, Units 1 and 2). This action was based on a significant breakdown in the licensee's corrective action 3rogram. The licensee identified deficiencies with bolts required for tie seismic adequacy of masonry walls in the emergency diesel generator building in 1987. However, the deficiencies were not evaluated until 1990, and astumptions used in that evaluation were inadequate.
The condition was not corrected until the NRC identified the problem in 1992, at which time the licensee shutdown the units. The civil penalty was escalated for NRC identification, prior opportunity for the licensee to identify, poor past performance, and duration.

St. Clares Riverside Medical Center, Denville, NJ paid the civil penalty in the amount of $10,000. This action was based on violations of NRC requirements which, in part, included failure to instruct persons caring for a patient with brachytherapy implants and failure to implement the radiation safety and quality management programs.
These violations contributed to a patient misadministration and an unplanned exposure to the hand of a licensed practical nurse at the facility.

The purpose of the meeting was to d;scuss arrangements for an upcoming Watts Bar emergency planning exercise presently scheduled for October, 1993.
Bi-National Hazardous Waste Work Group Meetina Charles Hackney, Region IV State Liaison Officer, participated in the Bi-National Hazardous Waste W1rk Group meeting on February 9-11, 1993 in Mexico City, Mexico.
The purpost of the meeting was to further the development and coordination of the Hazardous Waste Group activities prescribed by the Mexico \\U.S. Integrated Border Environme;.tal Plan.
Subcommittee on Water Resources (SWR) Meetina Maria Lopez-Otin, Federal Liaison, attended the Subcommittee on Water Resotrces meeting on February 10, 1993 at the Department of Interior.
The Subc m ittee was briefed by a representative from the United States Geological Survey on Spatial Data Coordination and by a representative from the Soil Conservation Service, Department of Agriculture on the third appraisal of the-Resources and Conservation Act.

Assistance to Armenian Reactors The European Community has awarded a contract to Framatome for a feasibility study of restarting the VVER reactors in Armenia.
Previously, framatome and EdF had perfsrmed a safety evaluation of these reactors in 1991. The contract I
is for two million French francs.
Incident at Russian Kola Nuclear Station The following information was received from contacts in Russia.
On February 2,1993 a storm at the Kola nuclear station in Russia damaged transmission lines and caused an automatic trip of Unit 1.
Three diesel generators connected to Unit'l started but did not come on line due to incompatible conditions on the internal station electrical-grid. -The diesels were repeatedly tripped by a signal from the generator failure-to-close breaker. The emergency actuation signal repeatedly restarted the diesels.
After three to four starts, the air accumulator was depleted and the three diesels became inoperable.
Approximately two hours after Unit I tripped, the other three reactors were shut down. There apparently was never a concern over core cooling, concerns were focused on the faulty diesel starting logic. The station instituted a procedural fix for the diesel starting logic and the reactors were restarted.
On February 16, 1993 Units 1, 2, and 4 were at power.
Unit 2 was down for an unrelated maintenance outage.
The Kola station consists of four VVER reactors--two 440/230s and two 440/213s. Units 1 and 2 have a total of five dedicated diesel generators.
Two diesels are dedicated to each unit and one diesel (#5) is a reserve, swing diesel for either unit. Units 3 and 4 have three diesel dedicated to each unit. There are a total of 11 diesel generators on the four-reactor site.
Fgreian Visitors On Tuesday and Wednesday Mr. Goran Steen, a contractor to the Swedish Nuclear Power inspectorate (SKI), and Mr. Bjorn Dufva of SKI met with the NRC staff to discuss NRC incident reporting, investigation and follow up, inspection philosophy and policy, pality assurance, and NRC action with respect to emergency sump blockage since the July 28, 1992 incident at the Swedish Barsebaeck 2 plant. They were also given a tour of the NRC Operations Center.
Mr. Steen, a former Director General of the Swedish Board of Inquiry, was appointed by SKI Director General Lars Hoegberg to conduct an independent-
-review of the Agency's actions after the Barsebaeck 2 incident and to formulate recommendations on how SKI can improve its inspection activities based on lessons learned from the incident. Mr. Dufva, Coordinator-of SKI International Affairs, accompanied Mr. Steen.

Region 1 Items of Interest Week Ending February 12, 1993 1.
Shioment of Scent fuel During the period of february 4-8, 1992 two shipments ef irradiated reactor fuel were made by Region I licensees. One shipment was from the MIT research reactor to a DOE facility. The other shipment was an export of irradiated reactor fuel rods from Ginna to France.
Both shipments were inspected at the point of origin and in-transit. No safety concern or violation of NRC or DOT regulatory requirements were identified during either shipment.

Three Mile Island The licensee declared a Site Area Emergency at 7:05 a.m., on february 7, due to a security event. An auto had crashed through a Protected Area gate and a Unit 1 Turbine Building roll-up door, and the driver had entered the building.
The NRC decided at 7:20 a.m. to go to the Standby Mode and staffed both the HQ and Region 1 Incident Response Centers.
Site security staff and state police apprehended the individual, hiding under the main condenser, about 11:00 a.m.
The licensee terminated the event at 4:35 p.m. after completing extensive security sweeps of Protected and Vital Areas. There was no impact on safety equipment or plant operation. The Deputy Regional Administrator, a Projects Branch Chief and several inspectors are on-site. The licensee has chartered an event review to be led by a Vice-President. The Commonwealth of Pennsylvania and the licensee have held several press conferences on the event. An NRC Incident Investigation Team (111) has been initiated.

Three Mile Island The incident Investigation Team (IIT) to review Sunday's Site Area Emergency security event held its entrance meeting at 10:00 a.m. on Tuesday, february 9.
The team leader held a press briefing later that day. The Governor of Pennsylvania issued a statement Monday evening, indicating that he was concerned with how this event could happen d
that he wanted his staff, including the Lieutenant Gevernor, to looK into a number of questions about plant security.

february 1 NRC Workshop on Erosion / Corrosion The NRC conducted an Erosion / Corrosion Workshop in Region I on february 9, 1993, with participation by most of the nuclear plant licensees nationwide and other interested parties. The objective of the workshop was to address current erosion / corrosion (EC) issues by promoting industry, NRC, and research organization discussions in regard to EC in high energy, carbon steel piping systems. . featured talks by industry, NRC, and research organization members relative to the implementetion of EC programs and breakout sessions

where major areas of ccacern were discussed. The workshop provided for an exchange of ideas on improving the practices of EC detection and mitigation between approximately 270 engineers, technicians, and managers in the Nuclear Power generation business.

International Visitors Mr. Victor Khromov and his interpreter, Mr. Andrei Glinianov, both from the Ukrainian State Committee on Nuclear and Radittion Safety, visited the Region this week. Their goals were to develop an understanding of the SALP process and how SALP boards are conducted, and to develop an understanding of how the NRC Regional offices integrate inspection findings into short range inspection planning. On Monday, February 8, they were accompanied by a representative of the Office of International Affairs.

Commission Meetinas On February 8, the Regional Administrator was in Headquarters to attend the Commission Meeting regarding Equal Employment Opportunity and to meet with the EDO in preparation for the Commission Meeting on February 9 concerning Operating Reactors and fuel Facilities.

Tennessee Vallev Authority On February 9, the Regional State Liaison Officer and a Division of Radiation Safety and Safeguards Branch Chief attended a meeting with officials from FEMA, the State of Tennessee, and the Tennessee Valley Authority to discuss Watts Bar emergency planning and the annual emergency preparedness exercise.

Commission Visits On February 10, Commissioner James R. Curtiss, accompanied by the Director, Division of Radiation Safety and Safeguards toured the Westinghouse Electric Corporation facility in Columbia, South Carolina.
On February 11, Commissioner James R. Curtiss, accompanied by the Director, Division of Reactor Safety, toured the Florida Power and Light Company's Turkey Point facility.

Duke Power Company On February 11, representatives from Duke Power Company's McGuire Nuclear Plant were in the Region 11 Office for an Enforcement Conference to discuss circumstances surrounding a violation regarding a recent event where a firearm came into the protected area undetected. Perry Public Meetino On February 10, 1993, a public meeting was held in Region III and attended by the Regional Administrator and other members of his staff, NRR, Cleveland Electric Illuminating, General Electric, the State of Ohio, and a local newspaper, the Cleveland Plain Dealer, lhe purpose of the meeting was to discuss a recent fuel rod failure at the Perry Nuclear Power Plant and licensee corrective actions.

Wiscorsin Public Service Corooratien On February 9, 1993, the Director, DRS, and other memberg of the Region III staff met with Charles Schtock, the Manager of Nuclear Engineering for the Wisconsin Public Service Corporation. A variety of issues were discussed. including M0V testing, seismic monitoring of the heating steam system, upcoming outage projects AFW pump modifications, and steam generator replacement (needed by 1999 for continued plant operation).

South Texas Pro.iect (STP) Auamented Inspection Team (AIT)
The AIT held a briefing meeting with the licensee on February 11, 1993.
The purpose of.the briefing prior to the exit meeting was to inform the licensee of ongoing status of the team's findings relating to the failure of both unit's turbine-driven auxiliary feedwater pumps. The team returned to-the region after the briefing. A team member will return to observe pump testing when the licensee begins it's testing program.
The team reviewed the licensee's progress in root cause analysis, corrective act. ions, refurbishment activitiu on the valves, postmaintenance testing procedures and methods, generic issues, and design problems.
The team concluded the root cause analysis was appropriate. Also, the team concluded the licensee's corrective action and plans are appropriate. The licensee made commitments to the NRC in a February 11, 1993, letter regarding generic reviews, programs to determine steam drain blockage on safety-related equipment, and testing programs.
NRC staff is meeting with the licensee in a public meeting at the site prior to the startup of the units, and an exit meeting is scheduled to be conducted in conjunction with that meeting.

River Bend Station Reoraanization The Vice President, River Bend Nuclear Group (the senior manager on site at the River Bend Nuclear Station) notified Region IV on February 9, 1993, that effective on February 16, 1993 the licensee's organization would be changed. -The reorganization changes the number of direct reports to the vice president from seven to four. The positions retained are Plant Manager, Engineering Manager, Site Support Manager, and the Manager, Sa.fety Assessment and Quality Verification. The direct reports to'these four managers have been increased to incorporate the functions previously reporting directly to the vice president.

Cooper Nuclear Station The-Deputy Regional Administrator and other members of the NRC staff visited the site on February 11, 1992, to attend an Outage Planning Meeting with licensee management. The-licensee is scheduled to begin a refueling outage on March 7,1993.

Decommissionino Status Meetina with Public Service Co. of_(plorado On Februery 10, 1993, Clegg Crawford, Vice President, Electric Production, Public Service Co. of Colorado,- and members of his staff, briefed tha Director Uranium Recovery Field Office (URF0) and selected members of the URF0 staff on the status of decommissioning activities at the Fort St. Vrain station.

Meetina with Pathfinder Mines Corporation On February 9,1993, the Deputy Director and selected members of the URF0 staff met with representatives of Pathfinder Mines Corporation to discuss a conceptual reclamation plan for the Shirley Basin Mill.

Meetina With Hydro Resources. Inc.
On February 10, 1993, the Deputy Director and two Project Managers, URF0, met with the Environmental Manager of Hydro Resources, Inc. (HRI)..
The meeting's purpose was to provide HRI an opportunity to reoort on their progress in preparing a revised Environmental Report related to the company's license application for its Crownpoint/ Church Rock, New Mexico, in situ leach uranium recovery facility. This meeting provided a general exchange of information.

Siemen's Power Corporation An Augmented Inspection Team (AIT) was dispatched to Siemen's Power Corporation, Richland, Washington. The~ team is comprised of Frank A.
Wenslawski, Deputy Director, Division of Radiation Safety and Safeguards, Region V (Team Leader), Charles Hooker, Region V,: and Mark Klasky,.NMSS.
The team's charter is to review the circumstances surrounding a degradation of criticality controls that occurred on. February 7,1993, involving a spill of 123 kilograms of 4.6 Wt. % enriched uranium oxide-powder.
The spill occurred when a limit switch, designed to stop_the-process flow of powder in the event of a line disconnect, f ailed _to function due to being taped over. The team plans to complete their inspection by Friday, February 12. 1993.
