- SIXTH INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlenjak on October 21, 1981 in accordance with 10 CFR 50.55(e) as NCRs BLN NEB 8113 through BLN NEB 8118. This was followed by our first interim report dated November 16, 1981.
Related NCRs BLN NEB 8119 R2, and 8205 and BLN NEB 8203 R2 were reported in our second interim report dated May 10, 1982; NCRs BLN NEB 8208, 8209, 8211, 8213, 8214, and 8215, and GEN QAB 8204 were reported in our third interim report dated August 16, 1982; and related NCRs BLN NEB 8217 and 8218 and BLN EEB 8203 R2, 8303, 8304, 8310 8311, 8312 8313, 8314, and 8315 were reported in our fourth interim report dated August 3,1983 Since that time, related NCRs BLN NEB 8221, BLN NEB 8302, BLN EEB 8402, BLN EEB 8405, BLN EEB 8413, BLN BLP 8328, and BLN MEB 8302 R1 have also been reported. Our fifth interim report was submitted on July 20, 1984. Enclosed is our sixth interim report. We expect to submit our next report on or about October 31, 1986.

't Bellefonte Nuclear Plant (BLN) and has issued three nonconformance reports (NCRs) since our last report, which identify additional equipment with qualification concerns. These NCRs are BLN NEB 8414, BLN NEB 8502R1, and BLN EEB 8501. The equipment identified by these NCRs, as well as unqualified or potentially unqualified equipment identified by the other "0588" NCR, is listed in Table 1, and actions being taken for these items are given in Table 3.
Items which have been determined to be qualified are listed in Table 2.
TVA will provide additional information by October 31, 1986.

BLN EEB 8308R2# Qualification documentation has been procured for the solenoid BLN EEB 8311R2# valves identified in these NCRs.
BLN EEB 8312R1' BLN EEB 8309R1 Current analysis has determined that the transducers listed on this NCR are located in a mild environment.
BLN EEB 8405 TVA has determined that failure of electrical cable for 1E circuits located in the east steam valve vault will not affect the ability to mitigate a steam line break and NUREG-0588 does not apply.
- These NCRs also appear on Table 1 as they also identify unqualified limit switches. TVA is discontinuing its reporting on the solenoid valves only.

BLN CEB 8119R2 TVA is in the process of relocating some of these transmitters to a less harsh environment per engineering change notice (ECN) 3022. The remainder of the transmitters will be in a mild, environment after relocation of the startup and recirculation system.
BLN NEB 8205R1 Devices identified in these NCRs will either be in a mild or a BLN NEB 8209R2 less harsh environment after relocation of the startup and BLN NEB 8211R4 recirculation system.
BLN NEB 8217R2 BLN NEB 8214R1 Some of these transmitters will be relocated by ECN 1603 to a less harsh environment while the remainder will be in a mild environment after relocation of the startup and recirculation system.
BLN NEB 8221 The valve operators will be relocated through ECN 2863 to an area above the maximum loss of coolant accident (LOCA) flood level.
BLN BEB 8414 TVA is in the process of obtaining replacement solenoid valves through ECN 2993.
BLN NEB 8502R1 This NCR identifies the fact that the three motors for the cpent fuel cooling pumps do not have qualification documentation, and TVA is evaluating these motors to determine if they can be qualified by analysis or if new equipment will be required.
i BLN EEB 8203R2 TVA is replacing these flex-conduit with stainless steel flex-conduit through ECN 1596.
BLN EEB 8308R2 TVA is in the process of replacing these level sensors.
BLN EEB 8308R2 Replacements for the limit switches identified in these NCrs BLN EEB 8311R2 are being obtained.

BLN EEB 8310 TVA is in the process of determining whether these tranducers can be qualified to their environment or if they will have to i
be relocated.
1 BLN EEB 8313R2 These devices are being relocated through ECN 2868 to an area outside the valve vault rooms with an environment to which the terminal blocks on the devices are qualified.
BLN EEB 8314R2 The identified handswitches are located in local test panels I
CF-ELPT-3-B and CF-ELPT-2-B. These panels are being replaced with qualified pendant station receptnoles through ECN 2868.

BLN EEB 8402 TVA is in the process of replacing these transmitters.
BLN EEB 8413 These magnetic starters have been relocated to a mild environment per ECN 3080.
BLN EEB 8501 Qualification for the cable trays listed on this NCR were done on a generic basis. However, the NRC informed TVA that this same approach used at WBN was unacceptable because TVA failed to prove similarity of chemical composition and manufacturing processes of the same cable types furnished by different manu facturers. TVA is now evaluating this problem.
BLN MEB 8302R1 TVA is still in the process of evaluating the documentation of these valve operators.
BLN BLP 8328 TVA has initiated ECN 3213 to add interposing relays between the limit and torque switches on the safety-related MOVs located inside primary containment. These relays will be located in the trained instrument rooms where the SSCS cabinets are loated and they will be insensitive to the low resistance values of the MOV leads.
