NRC INTEGRATED INSPECTION REPORT 50-413/96-12 AND 50-414/96-12
Dear Mr. McCollum:
On August 31, 1996, the NRC completed an inspection at your Catawba Nuclear
Station. The enclosed report presents the results of that inspection.
Violations of NRC requirements were identified by the NRC inspectors. The
first two violations involved the steam gene-
3r replacement secondary system
welding program and are of concern because
, indicate the failure of
programmatic barriers in welding program impL.nentation and welder training
areas that contributed to poor quality initial welds and very high weld reject
rates. The third violation involved the failure of design control
verification activities to identify that required screens in containment crane
wall penetrations and floor drains were installed to minimize the transport of
i
debris to the containment recirculation sump screens.
The violations are cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding the violations are described in detail in the
i
enclosed report.
Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
