SEVENTH INTERIM REPORT Description of Deficiency The following butterfly valves, manufactured by Ihsics Inflow Incorporated (BIF) a unit of General Signal Corporation, West Warwicko, Rhode Island, which are used in the Essential Air System were determined to be leaking in excess of an acceptable rate.
BIF Serial UNID Ntimber Mark No.
Contract No.
Item No.
Nucber 1 RJ-VTAC-50B 3BWO471-00-20 76K 38-86163-3 577 N-4 9445-10 1 RJ-VTA C-44 B 3BWO471-00-15 76K38-8 6163-3 576 N-4 9444-7 1 RJ-VTA C-27A 3 BWo471-00-15 76K38-86163-3 576 N-4 9444-2 Further investigation revealed that the rubber seats had failed because of damage, i.e., the seats were cut and portions were missing. The cause of the damage was not apparent.
Interim Progress All of the damaged valve seats have been replaced either by TVA or BIF.
TVA is presently planning a test program to spot check leakage rates on recently repaired valve seats to determine the adequacy of the replacement seats. IVA will provide the root cause and correspnding actions required to prevent recurrence of similar deficiencies in the final report.
