RHR PUHP MCff0R FAILURE The enclosed Inquiry Report from our Region III (Chicago) Office is forearded for your information.
The report relates to the failure of a motor which drives one of the four residual heat removal pumps and cracking of air diffusers in the motors for two other RHR pumps. A preliminary copy.of the enclosure was provided earlier to Jim Shea.
Ua have requested our Regional Offices to review the applicability considerations of this failure at other operating LTR facilities with RHa systems during their routine inspections. We will inform you of any significant findings.
