Subject: NRC Inspection of Palo Verde Unit Nos. 2 and 3

This refers to the special inspection conducted by Messrs. M. Peranich, S.
Stein, and K. Hooks of the Office of Inspection and Enforcement, and Messrs.
G. Hernandez and J. Ball of this office during the period of March 25-29,
1985, of activities authorized by NRC Construction Permit Nos. CPPR-142 and
143 and to the discussions of their findings held with members of your staff
,
during and at the conclusion of this inspection.
Areas examined during this inspection included new and spent fuel racks for
Unit 2 and Unit 3, and electrical cables, raceways and supports; piping and
pipe supports; and design change control for Unit 3.
Within these areas, the
inspection consisted of selective examinations of procedures and representa-
tive records, interviews with personnel, observations by the inspectors, and
examinations of hardware.
Based on the results of this inspection, it appears that one of your activi-
ties was not conducted in full compliance with NRC requirements, as set forth
in the Notice of Violation, enclosed herewith as Appendix A.
Your response to the Notice of Violation is to be submitted in accordance with
the provisions of 10 CFR 2.201 as stated in Appendix A, Notice of Violation.

Additionally, this inspection identified that certain of your activities
appear to deviate from licensing commitments made to the NRC, as set forth in
the Notice of Deviation, enclosed herewith as Appendix B.
Please respond to
this notice as requested in the Notice of Deviation.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure-
