APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Unit 1 License No. CPPR-120 As a result of the inspection conducted on April 15-18, 1984, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), published in the Federal Register on March 8, 1984 (49 FR 8583), the following violation was identified:
10 CFR 50 Appendix B, Criterion V states in part that " Activities affecting quality shall be prescribed by documented instructions, procedures,... and shall be accomplished in accordance with these instructions, procedures,..."
PSEG, Preventive Maintenance Procedure SA-AP.ZZ-010(Q) states in part that "all station department heads shall be responsible for....... ensuring the timely completion of required preventive maintenance on plant equipment..........."
Contrary to the above on April 18, 1984, the inspector identified numerous (approximately 400) safety-related "Q" preventive maintenance activities that had not been performed in accordance with the schedule for systems and equipment turned over from Bechtel Construction to PSEG startup/ operations and maintenance was past due for these activities.
Examples of past due maintenance activities are as follows:
Meggering of RHR pump motors.
Physical rotation of reactor feed pump shafts.
Confirmation of heaters energized in the emergency diesel generators.
l Cleaning of air filters for Channel D Vital Bus UPS Inverter.
Visual inspection of emergency diesel generators skid mounted components.
This is a Severity Level V Violation (Supplement II).

Pursuant to.the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within thirty days of the date.of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending this response time.
