Office.or Nuclear Reactor Regulation Items of Interest Week Ending September 4, 1992 Hatch Simulator Observation During the week of August 31, 1992,.a member of the Operator Licensing Branch and a member of the Office for Analysis & Evaluation of Operational Data observed the requalification examination simulator scenario validation at the Hatch nuclear plcnt.

This visi is part of a joint effort between NRR and AE0D to study the capabil@s.# various thermal-hydraulic and containment simulation models.

Issuance of NRC Bulletin 92-01. Supplement 1

.The staff issued NRC Bulletin 92-01, Supplement 1, " Failure of Thermo-Lag 330 Fire Barr.ier System to Perform its Specified Fire Endurance Function," on August 28,-1992, to all holders of operating licenses or construction permits for nuclear power reactors.

The'NRC considered recent test results of Thermo-Lag fire-barriers and determined that the 1-and 3-hour pre-formed assi klies installed on conduits, cabie-trays (of all sizes and configuratione:, and used to construct fire barrier walls, ceilings, and equipment enclosures do not p: ovide-the level of-safety specified by NRC requirements. Tests sponsored by TV Electric raised concerns relating to joint and seam ' separation leading to cable damage.

In' addition, they raise concerns about the potential for burn through of the Thermo-Lag material-itself.

Tests sponsored by the NRC appear to confirm concerns: relating to burn through of certain configurations of the Thermo-Lag material in the absence of joints and seams. The-staff issued this

-bulletin supplement:

(1) to notify licensees and construction permit holders of this new information associated with the Thermo-Lag 330 fire barrier system which many plants have installed to protect safe shutdown capability; (2) to request-all operating reactor. licensees that have Thermo-Lag fire barriers to take the recommended actions; and (3) to require that these licensees submit a

-written response to the NRC describing the actions taken.

VIB Insoection at Rotork Controls Limited. Balb. Enoland

'During: August 24-through 26, 1992, two members of the Vendor Inspection Branch (VIB) conducted an. inspection at the Rotork Controls Limited (Rotork Ltd),

facilities in Bath, England.

The purpose of the inspection was to follow-up on correc(ive actions-for findings identified during the 1989 VIB inspection and to review the status of the Rotork Ltd program for the dedication of commercial grade items. used in the assembly of nuclear valve actuators.

Rotork Ltd purchases parts then assembles, tests, and sells nuclear actuators and spare parts to U.S. utilities _through Rotork Controls Incorporated (Rotork Inc), Rochester,-New York.

These are sold as nuclear safety-related under l

their 10 CFR Part-50, App'endix B,-QA program to US utilities. Currently, nuclear actuators make up.only a small percentage of Rotork Ltd valve actuator sales.

As part of their corrective action to the 1989 VIB inspection, Rotork Ltd is in the' process of completing a commercial grade item dedication program for components used in their nuclear product line.

This program has been developed with consultant assistance and includes justification of materials SEPTEMBER 4, 1992 ENCLOSURE A I

2 used in their environmentally qualified actuators. While the program appears to prov.ide adequate guidelines for component / material dedication, specific imp _lementation_ requirements have yet to be developed.

It was also noted that iRotork Ltd.was not performing implementation audits of suppliers and manufacturers with ISO 9000 QA programs who are included in the United Kingdom Register of Quality Assessed Companies (DTI QA Register).

Technical Soecifications On August 31,~1992, the staff received the Tennessee Valley Authority's i

proposed technical specifications for their Watts Bar Nuclear Plant (WBN).

The proposed WBN technical specifications are based on Revision 0 for proof

and'reriew of the Westinghouse Owners Group (WOG) improved Standard Technical i

Specifications (STS). TVA currently has scheduled fuel load for WBN for January 1994.

In addition to their proposed technical specifications in accordance with the WOG improved STS, TVA submitted documentation describing requirements relocated-from their.1985 final draft technical specifications to

~ licensee controlled documentation in accordance with the Commission's Policy Statement on Technical Specifications Improvement.

Pressurizer Safety Valve Actuation at Ft. Calhoun

-On August 22, 1992, the pressurizer safety valve RC-142 actuated following a

-turbine trip transient. This same valve opened in early July and subsequently had problems reseating.

In the most recent event, the valve opened about 4%

below setpoint, i.e., at about 2400 psig, but did reseat as expected. The valve opened just below-the power operated relief valve (PORV) opening and

.high pressure trip setpoints.

.The licensee observed no leakage from the valve following the event and erformed several in-situ tests (via a testing assist device

'uced system pressure) which indicated both erratic readings and generall.,

. with the loop ' seals.

This is contrary to past observations on other valves-which have indicated little effect of loop seals on actual setpoint (i.e.,

.when the valve first-begins to open.) The licensee believes that the lower

-setpoint with the loop-seal-is due.to the. differential thermal expansion

-between the stainless steel nozzle and the carbon steel body.

Since the valves are set at thermal equilibrium conditions with steam, the licensee

' believes the lower temperature conditions affect the spring compression against the nozzle.

f t. Calhoun is the only plant which has _a carbon steel body type valve. This

. phenomenon is not currently believed to be generic since it has not been observed in tests on_other valve.models having stainless steel bodies.

To further investigate the setpoint phenomena and to inspect the valves for damage, the licensee has_ sent the valves to Wyle _ Lab. The licensee plans to establish valve settings and an uncertainty band which they will then use in their safety analyses. The licensee will perform these analyses pursuant to 10 CFR 50.59 and-is also going to lower the high pressure trip and PORV setpoints from 2400 to 2350 psia to further assure the safety valves will not open first.

transport of Nordico irradiators within the U.S.

The irradiators are Canadian-approved packages. Nordion is pursuing use of an overpack approved.

by the U.S. Department ot-Transportation for two irradiator models, and will request NRC certification for the third.

International U.S. Interaaency Tear. Visits Power Reactor On August h7,1992, the Salem and Hope Creek Nuclear Gencrating Stations provided a site tour for the U.S. Interagency Physical Protection Technical Exchange Team. The team consists of representatives of-the Department of Energy, Department of Defense. Arms Control and Disarmament Agency,.and the Nuclear Regulatory Commission. The purpose of the visit was to familiarize i

team members with physical protection measures in place at a typical U.S.

power reactor.

INDUSTRIAL AND MEDICA! NUCLEAR SAFETY Fu=1 Cycle-Safety Operational Safety Assessment - West _inahouse Electric Comeany. Columbia. South Carolina During the-period of August 17-28, 1992, an operational safety team composed lof representatives from Regions:1,LII, IV, and V; Headquarters; and the U.S.

Environmental Protection Agency _ assessed nuclear safety at the Westinghouse fuel facility, Columbia, ' South Carolina. Assessment reas included management controls,-chemical safety, fire safety, emergency p'.eparedness, environmental protection, health physics, and nuclear criticality safety.

Bonitored Retrievable Storaae (MRS)

On August-19, 1992, _the Division of Industrial Medical and Nuclear Safety _ and Division of Safeguards and Transportation -(SGTR) staff met with Department of Energy (DOE) Office of-Civilian Radioactive Waste Management staff-in a preliminary licensing technical exchange meeting to discuss physical

. protection for spent fuel storage at an MRS.

The licensing review process for the~ physical protection portion of an MRS license application was briefly discussed.

DOE indicated that it intends to develop an annotated' outline (A0)

2 for its physical security and safeguards centingency plans for the MRS similar to the A0 for the MRS license application and safety analysis report.

SGTR staff prosided DOE with its interim guidance for physical protection requirements for spent fuel storage in Independent Spent Fuel Storage Installations and discussed plans for rulemaking related to this guidance.

Detailed discussions on this interim guidance maybe the subject of future technical exchange meetings.

(pno Island Power Authority (Shoreham Nuclear)

On August 28, 1992, a meeting was held with the Long Island Power Authority (LIPA), and Pacific Nuclear Corporation concerning the disposition of spent fuel irradiated at the Shoreham Nuclear Station.

LIPA officials indicated that storage of this fuel in an Independent Spent fuel Storage Installation (ISFSI) was one of several options under consideration, as part of the over all decommissioning plan.

Several unique differences between LIPA and other facilities wer e brought up.

This fuel is of very low burnup therefore presents a low environmental hazard.

However, from a safeguards standpoint, it is not self protecting.

LIPA is currently holding discussions with the Pacific Nuclear Corporation, manufacturers of the NUH0MS* storage system.

LIPA proposed a very ambitious schedule for licensing of the ISFSI if it was chosen as the favored method.

LIPA also indicated it would pick a disposal option in the last quarter of 1992.

HIGH-LEVEL WASTE MANAGEMENT I

audit of Sandia National laboratories The Division of High-level Waste Management (HlWM) and Center for Nuclear Waste Regulatory Analyses (CNWRA) quality assurance (QA) staff observed a Department of Energy (DOE), audit of the Sandia National Laboratories Yucca Mountain Project (SNL-YMP) from August 24-28, 1992, in Albuquerque, New Mexico. The audit evaluated the.SNL-YMP QA program to determine whether it meets the requirements and commitments imposed by the DOE Office of Civilian Radioactive Waste Management. The SNL-YMP QA program was accepted by the NRC staff, with no exceptions, in October 1990.

The HLWM and CNWRA observers determined that the audit was effective, and agreed with the DOE audit team findings that SNL-YMP QA program implementation was generally adequate.

Three preliminary Corrective Action Requests were issued by the audit team, none of which is significant in terms of the overall QA program.

-This agreement defines the terms and conditions by which the USNRC and the UKAEA will continue cooperation in the area of thermal-hydraulic research.

The UKAEA has agreed to contribute $75,000 per year over the 5 year term of 1the' agreement and to exchange information and share their research results.

- The NRC will supply UKAEA with the latest released versions of the RELAP5 and TRAC-PWR codes.

Reactor (NP-HWR) Piping Integrity Review Group (PIRG) conducted-by the Oak

. Ridge National Laboratory for.the Department of Energy on August 27, 28, 1992.

Members 7of PIRG.have been assembled from industry, government and independent-sources to advise = DOE on piping design for=the NP-HWR. 'The objective of the review 11sito assist-DOE in placing adequate bounds on-the timing of a double-ended pipe breakiwhich_might: occur under dynamic seismic loading conditions, and to comment on the piping testing program now underway at ORNL.

- The SNL Surtsey-test' facility (1/10-linear scale model) and the ANL Corium Ex-Vessel -Interaction (COREXIT)' facility (1/40_ linear scale) are used to perform scaled experiments that simulate-hypothetical high-pressure melt ejection accidents in the Zion Nuclear Generating Station. These experiments are designed to-investigate the phenomena associated with DCH. High-temperature

instrument tunnel, and subcompartment structures. DePis is entrained by the steam blowdown into the Surtsey or the COREXIT, where specific phenomena, such as the'effect of subcompartment structures, water in the cavity, and hydrogen generation, can be studied.

At SNL, the Integral Effects Test (SNL-IET-8) was successfully conducted on August 26, 1992.

SNL-IET-8 was similar to SNL-IET-7, except that the cavity I

is half filled with water.

The peak pressure increase measured in the Surtsey vessel was 35 psi, whereas in ANL-IET-7 it was 40 psi.

The video camera located at the top port of the Surtsey vessel showed that the molten debris was able to reach this elevation, whereas in previous tests, the debris was not observed to. reach this elevation.

D' -iges to the structures (seal table room floor, crane wall, operating deck and refueling canal) were observed.

On the same, day, ANL successfully conducted an Integral Effects Test (ANL-IET-8). ANL-IET-8 is similar to ANL-IET-6 except that steam instead of N was used as an inertant in the initial atmosphere of the COREXIT facility. Tbe peak pressure increase measured in the COREXIT containment vessel was 19 psia, whereas in the ANL-IET-6 it was 37 psia.

It appears that steam is more effective than N in inerting the containment atmosphere.

Most of the 2

previous integral effects tests at SNL and all of the previous tests at ANL used N to simulate steam-in the containment atmosphere.

2 Division of Reoulatory ADDlications 1.LW TCC Meeting Sher Bahadur and Edward O'Donnell from RES and Leroy Person from NMSS participated in a meeting of the Host State Technical Coordinating Committee for Low-level-Radioactive Waste Disposal (1CC) in Palo Alto, California on August 26-27, 1992. The TCC is an organization focused on evaluating technical issues relating to the development of new low-level radioactive (LLW) dMposal facilities.

The TCC is composed of technical representatives from States legally designated t) hast LLW disposal facilities.

The ongoing activities in RES were reported as follows:

(1) a draft of the Branch Technical Position on Performance Assessment will be ready for NRC internal review in September 1992; (2) analysis and resolution of public comments on the Uniform LLW manifest rulemaking is underway; and (3) the final

. amendments to 10 CFR Part 61 which include a clarification relative to above ground disposal is undergoing staff review.

Vance and Associates summarized their work on quantifying the amount of I-129 an Tc-99 generated by commercial reactors based on measurements taken at operating power plants.

It appears that the quantities of these long-lived isotopes, which would exist long after the engineered portions of a waste disposal facility, have been greatly overestimated. The Vance and Associates methodology could be used'for performance assessment of future LLW disposal sites.

i. There were also presentations on:

(1) solidification technologies for both Class A waste and cement solidified material; (2) the use of EPRI's EPRINET system for electronic exchange of LLW information; (3) waste acceptance critoria for the proposed California disposal facility; (4) receipt, disposal, and record keeping at the proposed California disposal facility, and (5) development of a tracking system for the Central Midwest Compact.

The technical representatives from the $tates indicated that progress towa.ds opening new disposal f acilities is somewhat slos due, in great degree, to opposition from some members of the public.

Because of delays associated with siting and licensing of the new facilities costs are escalating.

For example, California is using the simplest technology in a dry desert crea and the developer has spent $40 million and still the construction has not yet begun.

Nebraska and Pennsylvania estimate that their facilities will cost $100 million to license and construct. However, no new technical issues came up at tts TCC meeting which would require Commission action.

Italdftn.LEtinonitfate,h_UEBJ Chief, IRB, and IRD staff presented a tour of the Operations Ce;.ter and engaged in a discussion of HRC's response to emergencies, including non-nuclear, to a large group of local officials from Chwila-Nam Province in Korea on September 1..The group's leader was Director of the Bureau of Civil Defense of the Province. Their main interest revolved around NRC's role.in relation to off-site cuthorities including local officials in siting a plant and in en.crgency response.

During this week, IRB continued to provide input to FDIA Situation Reports to the White House concerning Hurricane Andrew and its ef fects on nuclear power plants.

Technical Trainina Center (TTCl Jnternat'onal Workshon An international Workshop on the Conduct of Inspections and Inspector Qualification and Training was conducted in Chattanoogo August 30-Seltember 3, 1992. NRR was the lead office in arranging and supporting the workslop on behalf of the Work Group on Inspection Precedures-(WGIP) established by the Committee on Nuclear Regulatory Activities (CNRA) which is a technical committee of the Nuclear Energy Agency (NEA), Opening and closing plenary sessions for the conference were conducted at a Chattanooga hotel.

All other sessions, in the format of group discussions, were conducted using six classrooms at ti,e Technical Training Canter. Group discussions involving the various paiticipating countries cotered the major topics of conduct of r

inspections, training-and qualification of inspectors, and shutdown risks.

The conference was attended by 35 foreign and 20 U. S. representatives.

A. tour of the.TTC facility was arranged as part of the conference agenda.

The tour included each of the three existing full scope simulators configured to show different things. The GE sinailator was used to show an accident scenario and the use of emergency operating procedures.

This activity required the use of the recently added high fidelity simulator modeling.

The Westingnouse simulator was used to illuswate the static examination technique used in the evaluation of reactor inspectors as part of simulator training.

The B&W simulator was used to show how the various controls available at the simulator Instructor station can be-used to make the simulator'an effective tool for the training of inspectors. Also-included in the tour was an explanation and demonstration of some special training aids used in support of reactor technology-Waining.

The conference was ettended by personnel representing Belgium, Bulgaria, Canada, Czech and Slovak Federal. Republic, Finland, France, Germany, Hur. gary, Norway, Russia, Spain, Sweden, Switzerland, The Netherlards, Ukraine, United Kingdom USA, OECD/NEA, and_IAEA.

' workstation-based simulations that are planned for classroom use.

On August 24, 1992, the Document Control Desk (DCD) staff received from I

General Electric Nuclear Energy the GE Simplified Boil'.g Water Reactor j

Standard Safety Analysis Report (SSAR) for document processing and distribution. The submittal consisted of 52 copies of SSAR Chapters 1-21, four separate copies of Chapter-22 (17x22 engineering drawings), and SSAR proprietary information (separately wrapped).

The total volume of this submittal was 150 boxes of material delivered on four loading skids.

With the assistancelof Mail Room personnel, the DCD staff receipted and distributed the material within two hours of its arrival at the NRC. One copy was provided to the NUDOCS centractor for further processing.

ITS Trainina laboratory On August-21, 1992, the Information Technology Services Training Laboratory conducted 3 Wordperfect pilot training course entitled,

" Merging for Office froductivity."- The course is one of the new half-day training sessions designed to meet specific needs of agency users.

Performance Review Board during its review of SES performance appraisals and its recommendations for SES bonuses.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $12,500 was issued on August 31, 1992 to Sequoyah fuels Corporation, Gore Oklahoma.

The action was based on two events that occurred on May 1 and June 6, 1992.

In the first event, adequate procedures were not developed and adhered to for the safe conduct of a maintenance activity, resulting in a release of uranium hexafluoride

'(UF6) within the confines of the plant.

In addition, control room operators failed to properly.. respond to alarms signaling over-pressurization of the section of piping involved in the UF6 release.

In the second event, a control room operator failed to recognize and respond to alarms that signaled smoldering insulation on electrical heat tape around a nitrogen purge line on the Uf6 feed system.

Collectively, the violations associated with these events have been categorized as a Severity Level 111 problem.

The base civil penalty was escalated 150%

based on the following adjustment factors in the Enforcenent Policy:

identification, corrective action, licensee performance, and prior opportunity to identify.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amou'nt of $8,000 was issued on September 1, 1992 to Midwest Industrial 3

X-Ray, Inc., Fargo, North Dakota.

The action was based on NRC's inspection and investigative findings which found the licensee in violation of the following requirements applicable to its work in non-Agreement-States: 1) failure of radiography personnel to wear alarm ratemeters; 2) exceeding 180 days in NRC jurisdiction; and 3) failure to conduct complete surveys.

In _ view of the willfulress associated with the failure to wear alarm ratemeters, the violation was categorized at Severity Level'11.

The base civil penalty for Severity Level 11 is

$8,000.

The civil penalty adjustment factors were considered but no further adjustments were appropriate _ The remaining violations were classified at Severity Level IV and not assessed a civil penalty.

The' action was based on the licensee's failure to file NRC Form 241 and the Agreement State specific license with the NRC when performins licensed activity in non-Agreement States. The Radiation Safety Officer (RS0) was aware of the requirement in August 1990 and had copies of t he forms, but found it too burdensome.

The RSO stated he-intended to file the NRC form 241; however, the workload and pressure of other duties precluded him from filing.

Normally, the failure to c)mply with 10 CFR 150.20 is categorized at a Severity Level lIII; towever, due to the careless disregard for regulatory requirements exhibited by thr RSO, the violation has been categori.ed at a Severity level 11. The base civil penalty is

The action was based on the licensee's failure to 1) use a separate alarm ratemeter, 2) conduct complete radiation surveys, 3) post high radiation areas, 4) conduct a program for inspection and maintenance of radiographic exposure devices, containers, and source changers at intervals' not to exceed three months, 5) hold a certificate of compliance to transport Type B radioactive packages, and 6) have an NRC approved quality assurance program for transport of Type B packages.

Violations 1 and 2 were aggregated as a Severity Level 111 problem.

Based on w111 fulness, violation 3 was escalated to a Severity Level !!!.

The other' violations were categorized at severity levels IV and V and not assessed a civil penalty.

The base civil penalties were each escalated 50 percent since they were identified by the NRC, The penalty for-the Severity. Level'Ill problem was mitigated 50 percent for prompt and aggressive corrective actions.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of_$1250 was i: sued on September 2, 1992, to the Missourt Highway and-Transportation Department, Jefferson City, Missouri.

The action was based on the failure of the licensee's gauge user to follow the licensee's emergency procedures after damaging a gauge and the failure of the gauge user to keep licensed material secured or under constant and immediate' control.

The user left the area to obtain assistance not realizing the' source had broken off the device.

It was fortuitous that no member of the public appears to have handled the source during the

several hours it was unattended.

Each violation was categorized at-a Severity Level 111.

The base civil penalty was mitigated by 75 percent

for efforts in determining the root cause for the self-disclosing violations and for prompt and extensive corrective actions.

The

proposed civil penalty for the second violation was escalated 200% of the base civil penalty to convey a stronger regulatory message as

-provided for in Section VII of the Enforcement Policy.

On September 1, we distributed the hardline" furniture drawings of the work-stations and support and special spaces for floors two through ten to the prospective tenant' offices for their review and comment by September 22.

. tallow Continued Access The Southeast Compact Commission has decided to offer continued access to the Barnwell low-level radioactive waste (LLW) disposal facility to those States and Compacts which currently have such access by virtue of their compliance with the milestones of the Low-Level Radioactive Waste Amendments Act of 1985.

Access _for those States and Compacts is contingent upon successful negotiation of a contract with the Compact. Access will be permitted for the period January 1, 1993-June 30, 1994 after which time Barnwell will close to out-of-region waste. New Hampshire, Rhode Island, Michigan, Puerto Rico and the District of Columbia are not initially eligible for access.

Survey of Licensees in Florida and Louisiana State. Programs has contacted the State radiation control programs in Florida and Louisiana concerning potential damage or loss of radioactive sources as a

- result of_ Hurricane Andrew.

Florida is continuing to survey _ materials

. licensees in the damaged area, but has not identified any loss of control of regulated-material at-this point

Louisiana has completed its survey of its licensees and has not identified any loss or damage of radioactive sources.

State Programs will continue to monitor the situation.

TOM Training State Programs sponsored a workshop on Total Quality Management for Agreement State managers on September 2-3, 1992 in Nashville, Tennessee.

The workshop was conducted by Jack S. McGurk, Chief, Environmental Health Division, California Department of ilealth Services.

Participants toured Quadrex and SEG, LLW processing facilities, in Oak Ridge on September 1, 1992.

Meetina on Decommissionina of General Atomics Facilities Dean Kunihiro, Region V State Liaison Officer, participated in a meeting on the decontamination and decommissioning of General Atomics facilities on September 3,1992 in San Diego, California. Mr. Kunihiro discussed NRC's decommissioning requirements.

FCCSET point of-Contact Meetina Maria ~ Lopez-Otin, Federal Liaison and NRC's point of contact to the Federal Coordinating Council on Science, Engineering.and Technology (FCCSET), attended the contacts' meeting on September 4, 1992.

The purpose of the meeting was to discuss the full FCCSET meeting held the day 1before..The Chairman is a member of the full FCCSET. - The agenda for the meeting covered, among other things, presentations by the different committees on their budget crosscuts.

The scenario was very challenging and. involved simulated insider-caused sabotage leading to a large break LOCA.

FEMA observed off-site EP, and held a public critique on August 28. NRC Region I participated:in the exercise with site and base teams;.HQ participation was simulated due to Hurricane Andrew demands. ~ The NRC exercise inspection team concluded that the ability to protect the public was adequately demonstrated; f

Florida Power and Licht ComDany - Hurricane Andrew On September 1, there was considerable media interest in the condition of Turkey Point Units 3 and 4.

U.S. Senator Graham's (Florida) Office in Miami, Florida, questioned FEMA's Emergency Field headquarters in Miami considering calls from constituents who had heard of widespread radioactive releases, and that it would take years of work before the nuclear plants could be restarted.

Region 11 assured FEMA that this was not the case.

Region 11 inspectors had been on-site continuously during and subsequent to the storm monitoring licensee activities.

On September 2, considerable media interest continued in the condition of Turkey Point Units 3 and 4.

Region 11 developed a statement to be utilized by'NRC officials responding to inquiries from the public regarding Turkey Point's chronology during and subsequent to the hurricane and the results of radiological surveys at Turkey point.

On September 1, the' Turkey Point Nuclear Plant, Units 3 and 4, remained in cold shutdown after ex3eriencing damage from Hurricane Andrew.

Two sources of offsite power lave been restored to the site.

The' damaged fossil unit exhaust stack has been removed.

Extensive news media reports and inquiries continue.

North Carolina State Research Reactor Manaaement Meetina 0n: September 3.-the Director of the Division of Radiation Safety and Safeguards, the Region II Project Inspector and the NRR Project Manager conducted a management meeting with licensee representatives at the

-North Carolina State Research Reactor in North Carolina. The purpose of the meeting was to discuss _ status of operations at the facility.

In general, the facility is well run with strong management support.

The University considers the research reactor to be an essential part of the Nuclear Engineering Program.

Meetina with Private Citizen on Combustion Enaineerina (ABB)

The-Deputy Director, DRSS, and members of the Region III and HMSS staffs conducted a meeting on Septr.mber 3, 1992, with ABB Combustion Engineering, Hematite, M0 to discuss the licensee's plans for expanding

.their operations for making fuel elements at that location.

The licensee also indicated they are working with several Russian Fuel facilities regarding a potential joint venture. The details this venture are very preliminary and have included site visits by ABB emp?oyees to Russia facilities and Russian visits to the Hematite facility this week.

Subsequent to that meeting, the staff met with a private citizen in the area to address NRC's program for doing environmental assessment'and evaluating the proposed expanded program.

This citizen earlier had requested-a public meeting in the area (which was held-in April 1992),

and has.had subsequent communications with Region 111 on this issue.

LaSalle AIT An Augmented Inspection Team wts dispatched to the LaSalle Nuclear Power Station to review the circumstances surrounding a~ Unit 2 reactor scram on August 27.1992, Land several associated equipment failures.

A public exit meeting on September 3, 1992 was attended by state and local government representatives, members of the public, and several news reporters.

The team determined the difficulty with. tripping the reactor feet pump turbines was caused by contaminants in the hydraulic fluid and the safety relief valves had functioned properly but two position indicators had malfunctioned because of mechanical binding. The. turbine trip was caused by a problem with either the thrust bearing or its wear detector. The licensee is~ continuing its review of the event.

Oraanizational Realianment at Wolf Creek Generatina Station (WCGS)

Effective September 11, 1992, John Bailey, Vice President-Operations, was assigned to a corporate position with Western Resources, Incorporated (owner of 47 percent of WCGS) in Topeka, Kansas.

The position of Vice President, Operations was eliminated and the existing functions was reassigned to the Vice President, Engineering and the Director, Plant Operations. The Director, Plant Operations will report directly to the President and Chief Executive Officer of the Wolf Creek Nuclear Operating Corporation.

This organizational change is the latest in a series of planned organizational changes that began to be implemented earlier this year.

Unit 2 On September 1, 1992, the Assistant Director for Regions IV and V Reactors, Office of Nuclear Reactor Regulation, and tht Regional Administrator,.along with members of the-NRR and Regional staffs, met with representatives of TV Electric to review the status of construction cnd licensing activities for Comanche Peak Steam Electric Station, Unit 2.

Diesel Generator Damaae at Comanche Peak Steam Electric Station. Unit 2 On September 2, 1992, during the performance of acceptance testing of Emergency Diesel Generator B, Unit 2, a Senior Reactor Operator mistakenly attempted to synchronize the 7 MWe generator to the safeguards bus with the generator approximately 170 degrees out of phasc. _The event occurred at approximately 3:50 p.m. (CDT).

The licensee is coordinating with both the generator and diesel vendors to

= investigate the extent of damage, which-is not known at this time.

Office of the Executive Director for Operations Items of Interest Week Ending September 4, 1992 The EDD recently made a decision on a Differing Professional Opinion (DP0) concerning regulatory and technical specification (TS) requirements for electrical power sources. The decision involved initiation of the following actions by the Office of Nuclear Reactor Regulation.

1.

Clarify the relationship between the General Design Criteria and the exceptions to the GDC allowed by the Technical Specifications.

2.

Communicate the above clarification to NRC staff and licensees.

3.. Assure that the licensee responses to the yet to be issued generic letter on reducing risk during shutdown and low power operations either fully comply with the Model Technical Specifications presented in the-generic letter or, if not, that a valid safety evaluation be presented to juttify the position taken.

This should eliminate or provide a l

rational. basis for the Technical Specification differences from the GDC as identified in the DD9.
