Turbine Surp 011 Separator Outfall Code No: 800216 Discharge (00G)

Paraneter: Oil and Grease (Freon) Parameter Code No: 00556

1. Dates of Incident: 1/10/85 and 1/14/85
2. Identification of specific effluent limitation or monitoring requirenent affected:

Oil and grease daily maxi =um limitation of 20 zg/L.

3. Description of Incident:

Routine weekly grab sa:ples for oil and grease collected on 1/10/85 and 1/14/85 measured 72.2 eg/L and 56.1 mg/L. respectively.

4 Apparent Cause:

The new API oil / vater separator was re=oved from service for maintenance purposes. Tank T-41 oil separator re=ained in service during this period.

5. If not immediately corrected, the anticipated time the condition is expected to continue:

6. Corrective Action - The folleving corrective action was taken or vill be taken to reduce, eliminate and prevent recurrence:

The API oil / vater separator was placed back in service on 1/16/85.

7. Additional Comments:

The final discharge to Lake Michigan showed no visible signs of oil.
