1. Date of Incident: 1/31/85
2. Identification of specific effluent limitation or monitoring requirement affected:

Oil and grease monthly average limitation of 15 mg/L.

3. Description of Incident:

The monthly average oil and grease concentration for January 1985 measured 42.0 mg/L.

4. Apparent Cause:

The new API oil / water separator was bypassed because of a malfunctioning waste oil pump. A larger than normal amount of oil had accumulated in the turbine sump and was passed through to tank T-41 while the separator was out of service. As a result, the daily maximum limitation was exceeded on 1/21 and 28, 1985, as previously reported on February 4, 1985. These exceedances of the daily max resulted in the monthly average also being exceeded.

6. Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

As previously reported on February 4, 1985, a temporary waste oil pump was installed and the oil / water separator returned to service. In addition, the accumulated oil was pumped out of the turbine sump and Tank T-41. Also, a system engineer has been assigned the responsibility of designing and conducting a verification program for the new separator to optimize its performance.

The final discharge to Lake Michigan showed no visible signs of oil.
