Request for Additional Information Regarding Probability of UHI Isolation Failure: McGuire and Catawba Nuclear Stations, Units I and 2 In view of operating experience of nuclear plants with Upper Head Injection (UHI) systems, the NRC staff is conducting a generic review to assess the likelihood of loss of UHI auto-isolation capability, due to both human and equipment related common mode failures. Additional information for Duke Power Company's plants with UHI systems is needed to support this generic assessment.

Specifically, for each of the McGuire and Catawba units, we request that you provide information as to the demand and failure frequencies of the isolation ,

function such as acquired from test and maintenance activities (i.e., provide the demand and failure frequencies relating to challenges to the electrical and mechanical components of the UHI System upon actuation of isolation signals and identify the associated cause for each failure).

Our goal is to complete this assessment in late 1985. Your reply within 30 days of this letter would be consistent with our target completion date and is requested. Contact our Project Manager if you have questions regarding this request.

The reporting and/or recordkeeping requirements contained in this letter affect fewer than ten respondents; therefore, OMB clearance is not required under P.L.96-511.
