'Ihe operator discovered that valve V-26-17 was ocupletely pre-vented Ima opening and that valve V-26-15 una prevented frum opming vuore than 75% dae to the instalIation of Ex:affolding.
'Ihe operator secured the valves as far open an possible to facilitata purging.
