NRC INSPECTION

Gentlemen:
This letter refers to the inspection conducted by Mr. P. Qualls and Mr. A
McQueen of this office on October 5 through ']ctober 9,1992, of activities
authorized by NRC license Nos. NPF-41, NPF-51, and NPF-74, and to the
discussion of our findings held by the inspectors with members of your staff
at the conclusion of the inspection.
Areas examined during-this inspection are described in the enclosed inspection
report. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel, and observations by the inspector.
Based on the results of this inspection, one of your activities appeared to be
in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). Also we are concerned about your failure to classify the
reactor coolant system leakage event on December 23, 1991. We have made this
an unresolved item, After further review, this item may also be considered an
apparent violation. This violation and unresolved item are of concern because
of the importance of these items for protecting the public during an
emergency.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response,
in your
response, you should document the specific actions taken and any additional
actions you plan to prevent 1ecurrence.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure
will be placed in the NRC Public Document Room.
