SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION-REPORT-NO. 50-325/92-33-AND.50-324/92-33)

This refers to the inspection conducted by J. J. Lenahan.of this office on-

October 5-9 and 19-23, 1992. The inspection included a review of: activities

authorized for your Brunswick facility. At-the conclusion of the inspection,

the findings were discussed with those members-of your staff identified-in the

enclosed report.

Areas examined during the inspection are identified in the report. 'Within

these areas, the inspection consisted of selective examinations of. procedures

and representative records, interviews with. personnel, and observation of

activities in progress.

Based on th'e results of' this inspe'ction, certain of your activities- appeared -

to be in violation of NRC_ requirements, as:specified-in the-enclosed Notice of

Violation (Notice). The violation is of concern because of the apparent, lack

of management controls which permitted safety-related activities to continue

for more than- five weeks using an inadequate procedure. We are concerned-that

i

immediate corrective action was not implemented to revise and re-issue-

Procedure.WDP-002 after violation item 325/92-27-02 wasjidentified by NRC

Region II:.inspecto'rs on September 17, 1992.

Instead the Phase II structural

steel walkdowns continued using a procedure .which.did not comply with NRC

,

. approved visual welding acceptance criteria, and which did not adequately

address inspection of partial; penetration-welds.

You are required to respond-to .this __ letter and should follow the instructions-

specified-in the enclosed Notice when preparing your response.
