SUBJECT: NRC INSPECTION REPORT 50-482/92-31 (NOTICE OF VIOLATION)

Th'.s letter refers to the inspection conducted by Messrs. G. A, Pick and L. E.

Myers of this office during the period of October 11 through November 21,

1992.

The inspection included a review of activities authorized for your Wolf

Creek Generating Station. At the conclusion of the inspection, the findings

were discussed with you and those members of your staff identified in the

enclosed report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examination of procedures

and representative records, interviews with personnel, and observations of

activities in progress.

An unresolved item was identified during this inspection (Section 4.3) that

involved a failure to properly control heavy loads over spent fuel elements.

This event will be reviewed further to evaluate the work control process, as

well as complian , with the requirements of Technical Specification 3.9.7.

Based on the r

uts of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). Violation A pertains to an inodequate alarm response

procedure. This violation was identified by the NRC and is of concern because

the task group formed to evaluate a refueling water storage tank low boron

concentration condition failed to identify the procedure weakness

(Section 3.2).

Violation B pertains to inadequate postmaintenance test

instructions,

This violation is of concern because it is indicative of

continuing problems in the areas of work planning and control- (Section 3.6).

Violation C pertains to one example of failure to follow procedure that

resulted in decreased letdown flow and a loss of charging flow (Section 8).

This violation is of concern because it represents a continuation of a

negative performance trend involving operator inattention to detail.

You_r staff identified that a data transfer error of design sper.ifications

control i otor-operated valve-design information, which is a violation of

10 CFR 50,' Appendix B, Criterion 111. This violation was not cited because.

the criteria .in paragraph Vll.B.2 of 10 CFR 2, Appendix C (Enforcement

Policy), were satisfied,

lhis_ violation was an isolated occurrence, and the-

licensee staff took prompt and effective corrective actions (Section 3.6)'.

You are required to respond to this letter and should follow the instructions

~

specified in the enclosed Notice when preparing your response.

In-your

response, you should document the specific actions taken and'any additional _

. actions you plan to prevent recurrence. After reviewing your_ response to th_is

Notice, including your proposed corrective. actions and the results of_ future

inspections, NRC will determine whether further NRC enforcement actions is

necessary to ensure compliance Wth NRC regulatory ~ requirements.

In:accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

'this: letter and its enclosures will be placed in the NRC Public Document Room.

The response directed by this letter and the enclosed Notice-is not subject to

the clearance procedures of. the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
