NIAGARA MOHAWK POWER CORPORATION NINE MILE POINT - UNIT 2 DOCKET NO. 50-410 Final Report for a Problem Concerning Installation of Flexible Metal Conduit (55(e)-85-07)

Description of the Problem The problem concerns installations of type NWC and NPW Anaconda flexible metal conduits.

The installation specification specifies that in no case shall the

. bend radius of flexible conduit be less than the manufacturer's recommended minimum bending radius.

A review of a random sample of these installations identified that the bend radii given in latest manufacturer's data sheets and instructions were violated.

Construction and Field Quality Control were using an old version of the manufacturer's engineering data sheets.

Analysis of Safety Implications Failure to comply with the manufacturer's established minimum inside bend diameter could make the outer jacket of the flexible conduit susceptible to cracking, thereby affecting seal integrity of the conduit.

It could have resulted in the loss of safety-related ci rcuits.

Therefore, had this condition remained uncorrected it could have adversely affected the safety of operations of the plant.

Corrective Action Electrical Installation Specification No. NMP2-E061 A has been revised via Engineering and Design Coordination Report No. F02142.

In areas where it is not possible, due to space limitations, to meet the manufacturer recommended radii, it now allows the use of lower bend radii provided the flexible conduit jacket is sealed at the bend radius with Okonite jacketing tape.

The work already completed on Category I equipment prior to the revision of the specification, including the identified conditions, will be reviewed and evaluated.

Any nonconfonnances identified during this review, that are not acceptable-as-is, will be corrected to meet the revised specification.
