SIGNIFICANT CONSTRUCTION DEFICIENCY GE SUPPLIED GROUND BREAK RELAYS HOPE CREEK GENERATING STATION On November 28, 1984, a verbal report was made to Region I, Office of Inspection and Enforcement representative, Mr.

J.

Shedlowski, advising of a potentially significant construction deficiency concerning a manufacturing defect in ground break supplied by General Electric.

The following final report is provided in accordance with 10CFR50.55(e).

Description of the Deficiency Our Architect / Engineer and Constructor, Bechtel, has advised us that eight (8) Model TGSR-12 ground break relays supplied by General Electric may have a manufacturing defect in one constituent component.

The ground break relays were installed in Class lE 480 volt unit substations.

General Electric Service Advice Letter 175-9.12 addressed this concern.

Safety Analysis Bechtel has investigated the problem and determined that the suspect breakers were installed in 480 volt unit sub-stations 10B410, 10B420, 10B430, 10B440,.10B450, 10B460, 10B470 and 10B480.

Failure of these ground break relays could cause the incoming feeder breaker to trip to each safety related unit substation.

The unit substations identified above control various safety related equipment to include motor control centers in the Reactor Building, Diesel 8501250594 841231-PDR ADOCK 05000354 S

12/31/84 Generator Building and Service Water Intake Structure.

Had this deficiency gone undetected and uncorrected,

. failure of a. ground break relay could adversely impact safe operation of the plant.

Therefore, we consider this deficiency'to be reportable in accordance with 10CFR50.55(e).

Corrective Action Bechtel has replaced the defective relays with acceptable relays provided by General Electric.

Replacement was docu-mented on Quality Control Inspection Records 10B410-2-RW-1.00 through 10B480-2-RW-1.00.
