BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - DEFICIENCIES IN RECEIPT INSPECTION BLRD-50-438/84-27, BLRD-50-439/84 FINAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector P. E. Fredrickson on March 28, 1984 in accordance with 10 CFR 50.55(e) as NCR 2975. A similar deficiency was later documented and reported to inspector D. M. Vercelli on April 17, 1984 as NCR 3063 This was followed by our interim report dated April 25 and September 4 1984. Enclosed is our final report on both' NCRs.

BELLEFONTE NUCLEAR PLANTS UNITS 1 AND 2 DEFICIENCIES IN RECEIPT INSPECTION BLRD-50-438/84-27, BLRD-50-439/84-26 NCR 2975 AND 3063 10 CFR 50.55(e)

FINAL INTERIM REPORT Description of Deficiency NCR 2975 TVA's Quality Engineering Branch (QEB) released contracts with open deficiency reports, and receiving inspectors accepted material without verification of corrections or initiating a nonconformance report (NCR).

Also, receiving inspectors failed to perform required inspections or document such inspections. The cause of this deficiency was inadequate training of

the inspection personnel.

NCR 3063 The deficiencies described in NCR 2975 pertain to QEB-related contracts. The deficiencies in this NCR pertain to other contracts for which Bellefonte Nuclear Plant (BLN) site personnel have receipt inspection responsibility.

The deficiencies described in both NCRs are the same in nature.

Safety Implications Due to the deficiencies described above, it -is possible to have defective equipment installed and used in safety-related systems whose failure to perform its safety function would adversely affect the safety of the plant.

Corrective Action A review of receiving inspection checklists (RICs) that required source inspection has been completed. TVA's Office of Construction (OC) is verifying that site inspections were performed as required by QEB instructions for site inspections. A list of discrepancies that originated from the review has been categorized referencing material / equipment and the contract number. The discrepancies found have been evaluated and all corrective actions have been completed according to site procedures.

To prevent recurrence of this deficiency, all inspectors in rolved in receiving inspection were retrained and tested by supervisory personnel to assure adequate comprehension of the receiving inspection function.

In addition, BLN quality control procedure (QCP) 1.1, " Receiving Inspection,"

has undergone two major revisions to be more explicit for receipt inspection and inspection personnel.
