BELLEFONTE NUCLEAR PLANT INITS 1 AND 2 - IEE T INCDRRECT ADAPTORS (N INSTRtHENTATICN LINES - BLRD-50-43844-35, BLRD-50-43944 THIRD INTERIM REPORT The subject deficiency was initially reported to NRO-0IE Ir.spector P. E. Fredrickson on May 10, 1984 in accordance with 10 TR 50.55(e) as NCR 3102. This was followed by our interim reports dated June 4 and September 21, 1984.
Enclmed is our third interim report. We expect to suhnit our next report on or about July 3,1985.

_ THIRD INTERIM IEPORT Description of Deficiency A problem has been identified at Bellefonte which involvea adaptors (sized 3/4-inch and under) used to connect instrtment piping to instrtment tubing.
For tube-to-pipe adaptors with TVA Mark Nos. 142, 155, 153, 690, 691, 692, and T/8, the TVA mark ntaber or sch,edule is not affixed t3 the adaptor (they are rot required to be), and.the proper schedule adaptor required by design drawings may not have been used in all cases.
The apparent cause of this deficiency is the lack of schedule identifica-tion en the adaptors and.the fact that doctmentation of inspection verification of the adaptor's schedule is not required by present quality control procedures.
Interim Prcatress A myiew of all the weld maps concerning ASPE piping and the subject adaptors
'# has been completed and TVA is in the process of conducting a field verifi-cation of the adaptors. Upon completion of this verification a list of the adaptors with individual dispositions and the basis ased for determining each disposition will be prepared and myiewed.
