FINAL REPORT Description of Deficiency During installation of B&B Insulation Raarlex Sealant Material in six
.mechnical sleeves in the control building, two sleeves 5mre pumped with the Radflex material after the quality control inspector informed the crew that the 30-minute placing time had expired. Bellefonte Nuclear Plant (BLN) quality control procedure (QCP) 5.18 references B&B-QCP-1030.212 that specifies that both of the components of the Radflex Sealant material must be in place within 30 minutes of the start of the blending.
The root cause of this deficiency was due to an independent decision by the responsible craft supervisor to place the material in violation of the controlled procedure. The supervisor's decision resulted from frustration involving previous attempts. to' obtain relaxation of the blending time limit.
Safety Implications Willful disregard of QCPs by a supervisor whose responsibilities include ensuring QCP compliance by his subordinates is a significant quality assurance breakdown which could adversely affect the safe operations of the plant.
Corrective Action Correspondence with the vendor representative who handles the subject sealant material revealed that the 30-minute time limit requirement as referenced in BLN-QCP-5.18 and vendor procedure B&B-QCP-1030.114 is primarily a safety factor used by vendor personnel when a catalyst is used by them in their installations. BLN Office of Construction (OC) personnel do not use a catalyst in their installations. When a catalyst is not used the blending time to installation may be extended to 90 minutes according to the
- manufacturer's instructions. Therefore, the sealant material is acceptable as is.
l The craft supervisor has been given a written reprimand for his actions which reminded him of his quality assurance responsibilities and notified him that future instances would result in more severe disciplinary action.
