Fredricl: son on June 11, 1984 in accordance with 10 CFR 50.55(e) as NCR 3180. This was followed by our interim report dated July 10, 1984.

Enclosed is our final report. A several week delay of this submittal was discussed with Inspector P. E. Fredrickson on September 26, 1984.

FINAL REPORT l

Description of Deficiency The only full-penetration lug welds at Bellefonte Nuclear Plant (BLN) are on ASME class 1 piping. A total of 44 ASME class 1 lugs have been installed at BLN. A sample of 12 of these 44 lugs was ultrasonically n

examined and 8 exhibited discontinuities along the longitudinal axis of the lug where the lug intersects the surface of the pipe.

All of the 44 installed lugs are on the makeup and purification system.

This condition was previously idettified at TVA's Watts Bar Nuclear Plant (WBN) for ASME classes 2 and 3 full-penetration lug welds as nonconformance L

report (NCR) 5559 (WBRD-50-390/84-17, WBRD-50-391/84-17). TVA subsequently issued Quality Bulletin 84-10 to notify other TVA nuclear plants of the 7.

condition. As a result of an investigation into this item at BLN, the condition has been identified as applying to ASME class 1 lug welds at BLN.

l The cause of this deficiency has been determined to be a failure-by. craft personnel to consistently follow the requirements of TVA General Construction Specification G-29M. G-29M drawing 1.M.1.2-12 requires that the:1ug attachment welds be back ground or gouged before welding the second side.

Safety Implications TVA has completed a review of the class 1 piping analysis of the affected i

weld locations. The results of that myiew have been compared to

. applicable ASME Code requirements and indicate that the actual stresses

~ imposed on the affected lugs meet the requirements of the 1980 ASME' Code, paragraph NB3123.2. Thus, the configuration of the existing welds is considered adequate to meet the original design, and the NCR has been dispositioned to. "use as is." Theretcre, TVA no longer considers 10 CFR 50.55(e) applicable to this item.

1 To prevent mourrence of this deficiency, TVA has added an additional. hold point on the automated process control (APC) weld operation diecklist card for all ASME Code Section III, Class 1, full-penetration lug welds. This additional hold point will ensure that the backgrinding/ gouging operation.
