FIRST INTERIM REPORT Description of Deficiency During a review of all grout cards issued to install grouted anchors at Bellefonte Nuclear Plant (BLN), it was discovered that ntunerous anchors were not inspected to the requirements of BLN site quality control procedure (QCP) 2.8,
" Bolt Anchors Set in Hardened Concrete." However, it is' evident that some of these anchors had received inspection in accordance with specifications denoted on construction documents such as work releases or drawings.
The kpparent cause of this deficiency was a lack of understanding of QCP 2.8 by the applicable engineering units.
Interim Progress TVA is in the process of determining the extent of and the necessary corrective action for these conditions.
