of all documents that are in the boxen and transfer the boxes and all shredded material to the cuntody of B.A.

Meeting on 11/12/02 1 attended a meeting at Jim Pindley's office at 1155 P.M.
on November 12, 1982.
Attending the meeting was; D.

Chapman, B.
Canfield, J. Findley, and myself.
At approx. 2:00 P.M. throo n/A Q.A ouditors (vendor surveillence) arrived in M1, Pindley'n office.
The auditors reported that Zack people were shredding what appeared to be documents, including P.I.N.'s_(Pield Interference Notices) 4 I heard Mr. Findley call I.P.C.
security at approx.
2:15-2:20 although I did not hear the gist of the conversation.

It was not known as to the type of documents being

Due to recent r_umors of Baldwin Associates possibly taking control of HVAC. Jim Findley made a decision to get power pulled on the shreading machine. Tom Yearick was in route with security to pull power on the shreader at the_satte_ time Mr. Findlev W s discussing the matter with Corporate Zack.-
f.ieantime Dana Korneman contacted Chester Krey of Electrical to pull power on Zack trailer to keep anymore shreading. from taking place until Baldwin Associates could determine what documents were being destroyed.
After Mr..Findley comple-ted the discussion with Zack Corporate we~ went down to Zacks. complex to in-vestigate.
Before we left Jim found out power was'out at Zack and-stated to notify Chester Krey to restore power as the shreader had been shut down.
While at Zack Jim asked that Gary liosby be taken;from site.
Gary was talking.
to 7ach Corporate at this time and gave the phone to-Jim Findley.
This was-around_2:20 or 2:30 p.m. on llovember 12. 1982.
After coming back to office ate. Mere notified that Zack Corporate personnel was coming down to' talk-on

HVAC MEETING - NOVEMBER 12, 1982 Uhile attending a inocting in your office on Friday, November 12, the following occurred to the best of my reco11cetion.
Marsha Sumner, Jdin Moffatt and John Kent entered the trailer and reported that documents uere being shredded at the Zach trailer compicx.
On being ouestioned about what documents

were being shredded, their reply was that FIN's and other engineering documents were observed in boxes ready to be shredded, but they did not have a chance to see overything that was being shredded.
At approximately 2:10 p.m. Tom Yearick, BA Ast. Proicct Manager, entered BA0A trailer and was instructed to go wit h Ip Security and stop the shredding and secure all records to prevent further destruction.
Concurrently, a-decision was reached to cut off electrical power to the Zach trailers to insure that no further shredding would occur.
I instructed Chet Krey, BA E1cetrical Superintendent, to disconnect power to Zack's trailer complex until further notico, Tom Yearich, called me at approximately 2:20 p.tn. atid asi that power be restored, since he and security had successfully terminated all shredding and had posted security in the Zach trailer to prevent any further destruction of information.
I called Chet Krey and ordered that power be restored.
Following the meeting, J. E, Findley, R. J, Canfield, D. E. Korneman, U.-
Kent and G. Chapman toured the Zack trailer complex to familiarize ourselves with the areas used for storage of records and other documents.
The decision uns reached by all parties to necure three areas which contained documentation'and to taaintain IP Security on duty in the aren until Monday.

I make this statement freely with no threats or promises of reward having been made to me.
I am employed by RMC, Inc., as the Vice President for Sales, and I am a Registered Professional Engineer in the State of Illinois.
In April 1976 the Zack Company contracted with RMC to have RMC provice and install refrigerant piping at the Commonwealth Edison Company's LaSalle County
(
Huclear Station. The contract required RMC to install the piping on eleven air conditioning systems at the LaSalle-County Station.
Four of the eleven systems (Control Room Building, CR-1 and CR-2, and Auxiliary Equipment Electrical Room, AEE-1 and AEE-2) were specified in the contract as being nuclear safety related systems, the remaining seven air conditioning systems were not related to nucicar safety.
After reviewing the RMC files of the contract with the Zack Company at the LaSalle County Nucient Station, I found Zack had written to RMC on February 14, 1978, and had requested certifications on the material RMC furnished and

installed. Mic obtained certifications on the refrigerant piping on June 19, N
1978, and provided this information to the Zack Company on June 21, 1978.
On January 22, 1982, the Zack Company wrote to me at RMC and requested a letter
'i of conformance on the material provided at the LaSalle County Station. Zack's

Ictter of January 22, 1982, enclosed an example of the information 'they needed, which was:
"This is a letter of conformance certiying (sic) that the materials furnished by us for the refrigeration piping for the following systems are in compliance with Sargent & Lundy's J-2590 Technical Specifications and Sargent & Lundy approved Vendor / Supplier accessory data sheets. The systems are:
Service Building 1,2 and 3 (OVS); Off Cas Bu11dina (OVA);
Auxilliary (sic) Building Laboratory (OVL); Control Room 1 and 2 (OVC) and Auxilliary (sic) Electrical Equipment I and 2 (OVE)."
06 Since I knew the material certifications'were in the KMC files, along with copics of the Sargent & Lundy Specifications, I complied with the Zack Company request and furnished them the letter of conformance as they had requested, g RMC's letter of conformance to the Zack Company was dated January 22, 1982.

Shafer, Chief, Midland Section SUBJECT ZACK ALLEGATIONS MADE BY COVERNMENT ACCOUNTABILITY PROJECT I have reviewed the affidavits prehented_to this office by the Government Accountability Project (CAP) regarding the Zack Company.
One affidavit consisted of twenty-seven pages with forty-four exhibits,- (hereaf ter identified as A11eger 11), and provided the majority of information.
While reviewing this information,1 identified numerous documents-that appear to relate directly to the Midland Project (Attachment A). These allegations will be placed on the Midland Project Tracking System.
The exhibits presented by. Alleger H indicate that many of the problems have taken place in the past, took place during the RIII investigation in 1980, and may be continuing to date.
Even more important, however, is that Bechtel and therefore Consumers Power Company were aware of these problems and did not officially inform this office (Items 3-7 on Attachment A).
Based on this review I am recommending the following:
1.
Initiate a meeting with CPCo and Bechtel to discuss their involve-ment in Zack activities at Midland.-
2.
Expedite an: investigation / inspection regarding the twenty items in Attachment A.
3.
Initiate an immediate investigation to_ determine if Alleger 11 or any other alleger's rights were violated.
