Dear Dr. Grace:
Carolina Power and Light Company (CP&L) has received and reviewed the subject report and provides the following response.
Severity Level IV Violation RII 84-43-04-SL4 10CFR, Part 50, Appendix B, Criterion V states, activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished.
Contrary to the above, the procedures in Robinson Modification Package M-492-2, Revision 3, require the use of a washer when installing Phillips Red-Head Wedge Anchors. This washer was missing on hanger SW-8 25/16.
Additionally, workers were observed using a hammer to align a baseplate in SW-8 25/16 without an approved procedure.

Reason for the Violation One washer was omitted during the installation of a wedge anchor On hanger SW-8 25/16 due to the oversight of a craft worker. The QC inspection of this hanger did not identify the missing washer.
" Hitting an anchor bolt at right angles to it's axis witt a 48 ounce hammer" is considered to be within the " skill of the trade." A 48 ounce hammer, commonly called a three pound beater, is a tool carried by iron workers, mechanical workers, and is a general tool used by construction workers during the normal course of a working day.
It is not unusual for a worker to tap an anchor bolt, at right angles to it's axis, to ensure a proper fit of the base plate.
The installation instructions for Phillips Red-Head anchors entitled Anchoring Systems Specification Guide for Architects and Engineers states that anchors are to be driven and shows pictures of bolts being driven with a hammer.
3 Corrective Steps Which Have Been Taken The washer has been installed on the anchor, the bolt has been retorqued and accepted by QC personnel. This was completed on November 11, 1984.
The individual craf t worker responsible for the " missing washer" and the QC Inspector who was responsible for the inspection are no longer at the Plant site. The deletion of one washer out of the many installed is considered an isolated incident.

Corrective Steps Which Will be Taken Const-ruction Procedure WP-2 will be revised to describe the use of a beater in aligning anchor bolts. This procedure will specify that the angularity tolerance of 5% will not be exceeded and that the optimum torque values will be met.

Date When Full Compliance Will be Achieved Full compliance will be achieved by March 1, 1985, with the implementation of Construction Procedure WP-2.
