. Final Report for a Problem Concerning the Use of Nonqualified Breakers in Battery Chargers (55(e)-84-30)
Description'of the Problem An inspection of the Division III' battery chargers. 2BYS*CHGR2C1 and 2BYS*CHGR2C2 indicated that the de circuit breakers actually provided were different-from those shown on the bill of material.
The vendor was required to. provide qualified Class.1E circuit breakers manufactured by Westinghouse.
Correct dc circuit breakers were identified on the bill of material, but the
- battery. chargers supplied to the Nine Mile Point 2 site were equipped with nonqualified Square D de circuit breakers.
Analysis of Safety Implications s
Since the subject breakers were not qualified as Class 1E,.it is assumed that the breakers could have failed.
Failure of the circuit breakers would have resulted in the battery chargers not perfonning their intended function, thus eventually resulting.in a loss of Division III de power.
Therefore, had this problem remained uncorrected, it could -have adversely affected the safety of operations of the plant.
Corrective Action The.nonqualified Square D dc. circuit breakers will be replaced with nuclear-qualified de output breakers in accordance with Nonconfonnance and Disposition Report No. 8444 l
