Dear Mr. Hunter:

This letter provides the final 10CFR50.55(e) report concerning improper fattings found on the low pressure switch of the nitrogen accumulator for the main steam isolation valves (MSIV) at the Wolf Creek Generating Sta tion. This concern was initially reported by Messrs. Maynard and Creel of Kansas Gas and Electric Company (KG&E) to Mr. William Johnson of the Nuclear Regulatory Commission, Region IV, on October 25, 1983.

While removing teflon tape from certain fittings in the plant, an I&C technician discovered an improper fitting on the low pressure switch of a (MSIV) nitrogen accumulator. The female fitting on the switch, which was supplied as part of the valve by the vendor (Anchor-Darling),

was an AMINCO 1/4" O.D. tube connector that had (straight) threads. The male adapter fitting, also supplied by the vendor, was a 1/4" NPT pipe thread which utilized tapered threads. Use of these two unmatched fittings could have resulted in an inadequate pressure seal at the connection. Failure of this connection to hold pressure would result in a loss of pressure in the acculaulater which, in turn, would degrade the safety function of the isolation vz.lve.

An inspection of the other MSIV's and the main feedwater isolation valves revealed similar conditions in these valves. To remedy this situation, a transition piece was installed in the low pressure switch assembly of each affected valve. This corrective action has been completed, and the low pressure switch assemblies for the MSIV's and main feedwater isolation valves are now qualified for their intended use.
