E. Fredrickson on May 10, 1984 in accordance with 10 CFR 50.55(e) as NCR 3102. This was followed by our interim report dated June 4,1984.

Enclosed is our second interim report. We expect to submit our next report by February 24, 1985.

SECOND INTERIM REPORT Description of Deficiency A problem has been identified at Bellefonte which involves adaptors (sized 3/4-irch and under) used to connect instrument piping to instrument tubing.

For tube-to-pipe adaptors with TVA Mark Nos.142, 155, 158, 690, 691, 692, and 778, the TVA mark number or schedule is not affixed to tLe adaptor (they are not required to be), and the proper schedule adaptor required by design drawings may not have been used in all cases.

The apparent cause of this deficiency is the lack of schedule identifica-tion on the adaptors and the fact that documentation of inspection verification of the adaptor's schedule is not required by present quality control procedures.

Interim Progress TVA is still proceeding with the review of all the weld maps that concern ASME piping and the subject adaptors. To prevent this problem from recurring, hold points are being placed on the weld cards for the adaptors being installed to allow for a check of the adaptor's schedule, and Bellefonte Nuclear Plant Quality Control Procedure (QCP) 7.9 has been revised to require a quality control inspection to verify the correctness of the adaptor being used. TVA has also ordered special stamps so that an adaptor can be marked with its particular schedule.
