The hrbo Imbe Oil Pressure had boon decreasina since 2/1/34. Upon investigattens performed W Duke.

Delaval and Elliot representatives, it was determined that both the Right Front and Left Tront I Wrbochargers had esperiencmt thrust bearing failures.  !

The problem wcs as a result of insufficient tube oil being supplied to the thrust bearing faces during  !

startup. The oil flow was measured at .0211 sph per  !

~

turbocharger. After discussions with Delaval. the i eil flow was increased to .3427 sph per turbocharger. Further analysis is being performed by Dela' val to determine a solutten to thrust bes::ing failure. The turbochargers were replaced under WR1429FRF and WR1443PRF.

The 3L Cylinder Need started leaking jacket water on i 2/3/34. This fadicated a possible cracked cylinder j head. Upon removing the suspect head and pressuring j vith water, it was verified that a hairline crack had f developed. The crack was located in the area where  !

the Fuel Oil Injector goes through the head. The i cylinder head was replaced under WR1430PRT. The l; cause of the failure presently appears to 'ee a casting imperfection. Final resolution will be '

through NCI CN 067. f I

While the cylinder head was being replaced, the push {

rods were examined for cracks. Since 2 of 3 push i rods on 3L were cracked, all other push rods were l removed. Upon examination numerous push rods showed  ;

linear indications in the weld areas. A new version  :

of push rod is being manufactured by TDI which I utilises friction welding. This new type push rod was p1gced into the engine. NPR-CR-2169A was written [

to doctment this change. l l

While replacing the push rods, it was noticed that l the IR Rocker Box Assembly was damaged. The Rocker j Box Assembly holds the Rocker Arms in place for  !

engine operation. The damage was next to the [

Intermediate Enhaust Rocker Arm. H e entire Rocker  !

Box Assembly Jas replaced under WR1446FRF. The cause of the failu:e appears to be as a result of a ,

misalignment of the Intermediate tuhminst Rocker Aru l dowel pin. This miss11gsment probably occurred )

daring the Spring 1983 rebuild of this engiae. This  !

problem ha1 been documented se NCI CN-063.  !

1) he Turbocharger Lube Oil Drip Line orifices i were drilled to .070" under WR1455FRT. This i gLao orifice delivers .1427 aph of labe oil to  !

2) Right Saak hubosherger Ishoust'Healfelds helts had failed (4 total failures). Sase are 1/2"

- dianster belts of a mem-ferrous esterial. The bolts were replaced by other belts, manufactured by 131. H is problem will be desammated on a

- NC1.

3) A 3/8" 8.8. tubing line had been misaligned due to an impact caustas the line to rub against the i

engine. Vibration caused this line to wear through resulting da en air leak. This line is part of the Centrol Air Sys+w and is identified I as line E-53 (60 poi supply). The tubing was replaced under WR1439FRF. ,

J 2030 hrs 2/15/84 - Diesel Sonerater la restarted and leaded to 7000 EW.

Time Down - 40.5 bra temperatures en Jacket Water and Imbe 011. No apparent reason for discrepancies. This was subsequently resolved (soo 05:30 hrs - 2/23/84 entry).

1800 krs 2/20/84 - Diesel Generator 1A restarted and leaded to 7000 EV.

1640 hrs 2/21/84 - Diesel Generator 1A was vanvally shutdova again in an Accasulated Nours - attempt to determine why Outlet temperatures en Jacket  ;

S17 hours Water and Imbe 011 are high. No apparaat reason for ['

While the engine was shutdown, it was noticed that a 1/2" sapearew es. the 3R side door sever was missing its hapd. Upea replacing that bolt and retorquing the seter, another sapocrew head sheared off at less I than 15 ft-lb. The belts were replaced under l WR1515PRF. hrther feilare analysis is needed and i will be desumented by a NCI.  :

l 1700 hrs 3/9/84 - End of extended run test. Diesel Generator IA manually  ;

shut down. Total accumulated run time - 815 hrs. l 3/14/84 - As. a prerequaite for the Engineered Safeguard Features  ;

Actuation (ESFA) Functional Test, the Diesel Generator IA  ;

vas manually started. The engine was manually shutdown l' after 1 minute due to a clogged fuel s trainer. After cleaning the strainer, the Diesel Generator IA was '

restarted and verified as ready for testing.  ;

3/16/84 - During the ESFA Functional Test, the Diesel Generator IA did not respond to an emergency start signal due to a faulty relay in the Diesel Generator IA load sequencer.

Investigation into the non-response revealed that the relay had burned out due to a temporary modification l installed in the sequencer logic system during checkout.  !

This modification was rewired and the relay replaced.

1950 hrs 3/17/84 - Diesel Generator 1A restarted for 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> full load capacity test.

l 0330 hrs 3/19/84 - Full load test satisfacterily completed. Diesel l Generator IA manually shutdown.

2000 hre 3/19/84 - Diesel Generator 1A manually restarted to warm-up for ESFA  ;

test.  !

2201 hrs - Diesel Generator IA manually shutdown. I 2204 hre - Diesel Generator 1A received ESFA signal. The Diesel Generator IA responded and ran for 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, then was  ;

manually shutdown.

3/20/84 - The Diesel Generator IA was manually restarted to calibrate test equipment. After 2 minutes, the engine ,

tripped due to leaking fittings on the control air system i (causing spurious trips). After 6 additional starts, the ,

leaking fittings were located and corrected.  !

l 3/21/84 - The Diesel Generator IA was restarted for ESFA test. The i

Diesel Generator IA ran satibfactorily for 1 hr at 7700 l KW and then manually shutdown.

The Diesel Generator IA l was again restarted and run satisfactorily for 1 hr at 7000 KW.

[ 3/22/84 - The Diesel Generator IA war Testarted on a simultaneous '

t blackout and LOCA signal. The Diesel Generator IA was run satisfactorily for 66 minutes at 4100 KW (full LOCA '

load) and then manually shutdown.

3/26/84 - The Diesel Generator IA was manually restarted and run i for 1 he at no load to calibrate test equipment, then manually shutdown.

3/27/84 - The Diesel Generator IA was restarted via ESFA signal and run for satisfactorily 1 he at no load, then manually shutdown.

3/29/84 - The Diesel Generator IA was restarted for blackou t testing. After approximately 2 minutes, the Diesel Generator IA tripped and showed no trip indications. -

3/29 3/30/84 - During this period the Diesel Generator IA was started an additional 21 times as part of the troubleshooting of the ,

cause of the trip described above. The " TIMER /NOT 9" '

pneumatic module on the shutdown logic board had drifted out of calibration, causing the engine to trip after 2 minutes. . The module was recalibrated . The Diesel  ;

Generator IA was then restarted and run for 15 minutes for the blackout and load rejection test at 7000 KW, then '

manually shutdown. The Diesel Generator 1A performed i I

l satisfactorily af ter recalibration of the module.

3/31/84 - The Diesel Generator IA was restarted and run for blackout testing. The Diestl Generator ran  ;

satisfactorily for 15 minutes at 3000 KW, carrying i blackout loads only.

l 4/02/84 - The Diesel Generator IA was restarted and run for blackout and load rejection tests. The Diesel Generator IA ran satisfactorily for 1 br at 7000 KW, and was then ,

manually shutdown. l 4/03/84 - The Diesel Generator IA was manually started and run for 15 minutes at no load, then manually shutdown. The Diesel Generator 1A was again restarted and run for 40 minutes at no load, then manually shutdown.

After an accumulated 857 hours0.00992 days <br />0.238 hours <br />0.00142 weeks <br />3.260885e-4 months <br /> of operating time, disassembly of Diesel Generator IA began for its detailed inspection of the engine. The results cnd conclusions of this detailed inspection are contained in the report cntitled " Catawba Nuclear Station, Diesel Engine 1A Component Revalidation Inspec tion, Final Report" dated June 29, 1984, which was submitted to the l Nuclear Regulatory Commission on that date.

B The following is a listing of significant events during the Diesel Generator j 1B Ratended Run:

1700 hrs 5/30/84 - Diesel Generator IB started and loaded to 7000 KW.

2250 hre 5/31/84 - Tbe Diesel Generator was manually shutdown by the shift  ;

Accumulated hours supervisor because of oscillations in the voltage  !

281.5 bra regulator. After evaluations were performed, the oscillations were not considered to be detrimental, and l the Diesel Generator was approved for restart. l 0050 hre 6/01/84 - Diesel Generator 1B was restarted and loaded to 7000 KW  !

0720 hrs 6/03/84 - Diesel Generator it was annually shutdown due to a leak  !

I Accumulated hours on the right bank aftercooler inlet jacket water line i 335.5 hrs. coming from a crack between the casing and the flange.

Preliminary estimations of the cause of the crack was i vibrat ion. The aftercooler was weld repaired and j l returned to service under Work Request No. 9822 OPS. In '

addit ion, both turbochargers were replaced because of
excessively worn thrust bearings caused by the inadequate pre-lube during starting. (Work Request No. 9794 OPS) A (

redesign of the turbocharger pre-lube system is currently i in progress, which will provide a greater flow to the '

l turbocharger thrust bearings. Also, the 7R fuel pump was

! replaced due to low pyrometer readings on this cylinder (WR 2180 PRF). This corrected the low temperature  ;

problems when the engine was restarted.

2021 hrs 6/12/84 - Diesel Generator 15 was restarted and loaded to 7000 RW.

0817 hrs 6/15/84 - Diesel Generator 18 tripped on low lube oil pressure. i This occured as operations personnel were attempting to j change the lube oil strainers while the Diesel Generator was on line. (

While the engine was down, the 5L fuel injection pump was  !

replaced due to low pyrometer readings on this cylinder '

(WR 2259 PRF). At the same time, the 5L fuel injector was  ;

l checked under WR 2262 PRF. It was determined that the i injector was the cause of the problem rather than the '

fuel injection pump. The injector was replaced. l It was noted that a 1/2" capscrew on the right bank  :

starting air distributor was sheared off. This capscrew I was replaced under WR 10023 OPS. The probable cause of l the capocrew failure was engine vibration.

6/15/84 - 6/21/84 - Diesel Generator it was started and stopped several times f during this period while repairing and adjusting the  ;

0740 hrs 7/01/84 - Diesel Generator 1B was manually shutdown because of Accumulated hours jacket water leaks in cylinder heads 8R, 7R, and 4L. 8R 616 bra cylinder head was replaced. Cylinder heads 71 and 4L were considered to be acceptable leaks, not detrimental to the engine. The cause of the crack in the 81 head is currently under investigation.
