Dear Mr. Starostecki:

The following is submitted in response to your letter dated April 16, 1984 concerning the violation identified in Inspection Report No. 50-410/83-16.

Niagara Mohawk Power Corporation agrees that the combination of weld symbols shown on Drawing No. NMP-023 and Engineering Change Notice (ECN) No.01-003 could be subject to interpretation. The intent of issuing ECN No.01-003 was to provide an alternate (and acceptable) welding configuration for hard to access areas by using a combination of the two welding methods, if needed.

In order to demonstrate that the identified welds were installed in accordance with design requirements, Stone & Webster Engineering Corporation and Reactor Controls, Inc. performed a reinspection of the subject field welds for Reactor Controls Inc. supports depicted in Drawing No. NMP-023, Sheets 1 and 2, Revision 1.

All welds were acceptable.

Revision 2 of drawing No. NMP-023 has since been issued to clarify the weld symbol. Action to prevent recurrence is provided in the Niagara Mohawk l

Power Corporation response to the Construction. Appraisal Team (CAT) Audit, dated May 4, 1984, alleged violation B, page B-14.

Specifically, Reactor Controls Incorporated Quality Assurance Instructions (QAI) 8-2, 8-3, and 10-1

'have been revised to provide detailed instructions for installation activitics l

and inspection requirements. Seven Quality Control personnel and installation supervisors have been trained to these revised procedures to help assure that inspections are adequately performed and training has been provided to supervisory personnel to help assure a thorough understanding of acceptance requirements.

In accordance with the resolution reached in a telephone conversation among Mr. R; Gramm (NRC Resident Inspector) and Mr. T. Loomis (Niagara Mohawk Nuclear Licensing), this item will be discussed during a June 5-7, 1984 meeting in the Stone & Webster Cherry Hill Operations Center between Niagara Mohawk, General Electric and the Nuclear Regulatory Commission Mechanical Engineering Branch personnel.
