On March 8, 1984, it was discovered that technical specifications required actions concerning performance of maintenance on Reactor Water Cleanup System, primary containment isolation valve 2-G31-F004 were not met.
LER 2-84-5 was to have been submitted to your office by April 6, 1984. However, investigation and evaluation of this event is not yet complete.
It is anticipated that this report will be submitted by May 7, 1984.
