In response to your letter dated January 9, 1984 which transmitted the subject Inspection Report, the attached response to the cited item of noncompliance is provided.

Violation 10CFR50, Appendix B, Criterion V, as implemented by Duke Power Company Topical Report, Duke 1-A, Section 17.2.5 and Duke Power Company Adminis-trative Policy Manual for Nuclear Stations, Section 3.3.2.3 requires that maintenance activities affecting quality be performed in accordance with written procedures.
Contrary to the above, during the Unit 2 Cycle 7 refueling outage, mainten-ance activities affecting quality performed on the 2B2 reactor coolant pump

were not accomplished in accordance with written procedures, in that seal staging coils were not installed per written procedure TN/2/A/1542/4 re-sulting in the operation of the 2B2 reactor coolant pump with the seal leakoff path blocked.
This is a Severity Level V Violation (Supplement 1).
Response
1)
Admission or denial of the alleged violation:
This violation is correct as stated.
2)
Reasons for the violation:
This violation resulted from personael error. Maintenance personnel used incorrect verbal directions for locating the seal staging coils instead of referring to the procedure enclosure which showed the correct locations.
3)
Corrective actions taken and results:
Investigation of abnormal seal leakoff indications following unit startup revealed the blocked leakoff path caused by the incorrect staging coil location. The unit was shut down and appropriate repairs were made, including correcting the staging coil locations and replacing the seal packages on the 2B2 RC pump.
The Maintenance personnel involved with the incorrect seal staging coil installation were counseled. The procedure used to replace tne staging co!.ls has been changed to more clearly show the correct coil locations.
4)
Corrective actions to be taken to avoid further violations:
This event is being reviewed in detail by all appropriate Maintenance personnel in the Mechanical Maintenance and I & E Sections.
5)
Date when full compliance will be acheived:
This review noted in (4) above will be completed by March 1, 1984.
