Dear Dr. Murley:
JCONSTRUCTION DEFICIENCY GE TYPE AKR-30 CIRCUIT BREAKERS HOPE CREEK GENERATING STATION On December 23, 1983, a verbal report was made to Region I, Office'of Inspection and Enforcement representative, Mr. W.
Cook, advising of a potentially significant construction deficiency concerning the failure of several General Electric AKR-30 circuit breakers to operate properly.
On January 18, 1984,'an interim report was sent to your office. 'The following supplemental information is provided in accordance with 10CFR50.55(e).
During on-site rework of the subject AKR-30 circuit breakers, performed under the guidance of General Electric representatives, additional fabrication deficiencies in the breakers were identified, including:

Loose buffer assembly hardware 2.
Short motor spacers 3.
Broken solder joints 4.
Defective spring charging motor b;:
5.
Broken driving pawl shear pin l4 6.
Improperly installed cut off switch plate K.
7.
Cut-off and closing circuit switches becoming loose in

Murley-2 2/24/84 Consequently, General Electric has agreed tu) ' completely reinspect all~ Hope. Creek AKR-30 circuit breakers in accordance with detailed
!. inspection procedures reviewed by Public Service and our Architect /
- Sngineer, Bechtel-Power Corporation.
We'have also instructed'Bechtel to press General Electric to identify the cause of the breakdown in their quality program that-permitted these_ fabrication deficiencies.to leave their shop.
-- An additional interim report will be provided by May 14, 1984, if
.the final' report is not available' prior to that date.
