BLRD-50-439/83 THIRD INTERIM REPORT The subject deficiency was initially mported to NRC-0IE Inspector P. E. Fredrickson on September 1, 1983 in accordance with 10 CFR 50.55(e) as NC3 837-66 Deficiency No. 2.
This was followed by our interim reports dated September 26 and December 19, 1983 Enclosed is our third interim repor t.
We expect to submit our next report by June 15, 1984.

THIRD INTERIM REPORT Description of Deficiency Vendor audit 83V-66 was conducted _ at Atwood and Morrill Company, Salem, Massachusetts, during the period of August 8-11, 1983 As a result of this audit, deviation 83V-66-2 was initiated which identifled the lack of adequate quality assurance requirements imposed on purchase orders for the manufacture of class 1E solenoid valves and air-operated valve actuators. TVA contract No.
82K21-830319, Appendix B, " Quality Assurance Requirements for ASME Section III Valves, requires ANSI N45.2, " Quality Assurance Program Requirements," to be imposed on the valve actuator manufacturer. No cbjective evidence was available to indicate that the purchase and manufacture of air operators were in accordance wit?. contract requirements.
Interim Progress Valves for the noted contract have not been shipped. A hold on Atwood and Morrill equipment was initiated by TVA's Division of Purchasing on September 2, 1983 An audit of Komox Corporation, Tufline Division, Cincinnati, Ohio was conducted during the period December 6 and 7,1983, by the Atwood and Morrill quality assurance manager with surveillance performed by TVA's Office of Quality Assurance Design Quality Assurance Branch personnel. This review and audit of Tufline Division's quality assurance program determined that adequate controls were implemented for the manufacture of air actuator subassemblies.
Inspection and test activities will be performed by Atwood and Morrill on the assembled actuators to verify conformance to specification requirements. These tests will be documented and witnessed as required by TVA's Division of Engineering Design (EN DES) Quality Engineering Branch (QEB). The release of the valves is subject to acceptable TVA inspection results.
