E. Fredrickson on March 7,1983 in accordance with 10 CFR 50.55(e) as Audit 83V-10 Deficiency No.1.

This was followed by our interim reports clated April 5 and August 26, 1983 Enclosed is our third interim report. We expect to submit our next report by January 14, 1985. We consider 10 CFR Part 21 applicable to this deficiency.

AUDIT FINDING 83V-10 DEFICIENCY NO.1 10 CFR 50.55(e)

BLRD-50-438/83-24, BLRD-50-439/83-19 THIRD INTERIM REPORT Description of Deficiency During TVA quality assurance audit 83V-10 at Bingham-Willamette Company (BWC), Portland, Oregon, three quality assurance program deficiencies were identified. The items being supplied to TVA are steam turbine-driven and electric motor-driven auxiliary feedwater (AFW) pumping units with spare parts.

Out of the three audit deviations identified as a result of this audit only deficiency No.1 was considered a significant condition adverse to quality.

This deficiency noted that BWC had supplied and was currently in the process of supplying ron-ASME code (noncode) items that were not manufactured in accordance with a documented quality assurance program as required by TVA contract No.

76K31-86133

.It was determined that this deficiency was a result of BWC inadvertently overlooking contractual requirements for compliance with ANSI N45.2 for all noncode items during their initial breakdown of the above contract into BWC working documents. These working documents indicated that original noncode items supplied to TVA required no quality assurance documentation; therefore, when work was started for spare parts ordered on Contract Change No. 9, they were identified as requiring no quality assurance documentation and manufacturing began under BWC's commercial shop practices.

Interim F" ogress Since the L'at interim report, TVA has been actively ' investigating several possibilities for correcting the problems at BWC in an economical and expedient manner. To date, the possibilities have been narrowed to the following two options:

1.

Test the pump internals and, if satisfied with the results, present them to NRC for approval as being " equivalent" to the required documentation per the specification. The test would consist of the following steps; a.

Perform spectrographic analysis on each impeller, shaft, and shaft sleeve to verify the diemical composition of the materials supplied.

b.

Excavate one or more cavities in the usual locations of weld repairs (as indicated by BWC experience) on one of the scrapped impellers from the spare parts order.

c.

Perform weld repairs, restore the surface to design conditions by grinding and postweld heat treating and perform both nondestructive and destructive examination.

If tha abova stcps establish the fGasibility of conducting nordestructive cxamination (NDE) on tha impallars and othre internals in the pumps, then it'chfi,,

be determined if they have been weld repaired and, if so, checked for cracks which may result from postweld heat treatment or improper heat treatment. If this procedure proves feasible, the original impellers, as well as other pump internals, can be tested by NDE to determine acceptability and conformity of the materials and workmanship with relation to the contract specification.

2.

Require BWC to remanufacture the original pump internals and spare parts.

These options were formally presented to BWC in a letter dated January 17, 19814.

. TVA is awaiting BWC's response to these options before taking any additional action.
