The RHR containnent spray outer valve operator (M0-2-10-26A) failure.

Conditions Prior to Occurrence:

Reactor shutdown with primary coolant temperatures less than 212 F and reactor pressure vessel vented.

J Description of Occurrence:

During 'a plant outage, a routine inspection showed that the subject valve motor casting had broken away from the valve operator.

The motor was still attached to the operator by way of its output shaf t.

Designation of Apparent Cause of Occurrence:

Improper support of valve, coupled with line vibration, re-sulted in breaking of the motor end bell casting.

Analysis of Occurrence:

The RHR system is equipped with two individual sub-systems capable of providing a water supply to the drywell spray headers. The inability to open a single valve to one of the spray headers does not prevent the operation of the containment spray-system. These valves are utilized following an accident which results in steam accumulating in the drywell and are manually operated. There is no logic associated with these valves. This occurrence, therefore, has minimal safety significance.

Corrective Action:

Tine vaive instaliation was inspected. uuring tnis. inspection I it was noted that the valve operator.was,not properly bolted to the yoke assembly.. Corrective action consists of properly aligning the valve operator with the yoke, replacing the motor on the valve operator, and adding additional support to the valve and its operator.

Failure Data:

No previous failures have been reported on this valve.
